Patient Connect Portal

Patient Connect Portal is a secure, digital platform that helps existing UCI Health patients or people that received imaging at UCI Health easily access and own your medical records and images.  

Create a history of your health record to help manage routine or long-term care. With Patient Connect, you have one place to collect, store and share all of your medical records and diagnostic images to help bridge any gaps between you and your doctors. 

You can help keep your doctors informed by understanding your unique health history, or if you have a chronic illness or require care across different doctors, you have a centralized location to compile all this data. Your health history is organized and follows you wherever you seek care.

Register at the Patient Connect Portal sign-up page or watch this short video for step-by-step instructions. Registration for a personal account is simple and only takes a few minutes. After you answer a few questions, you can start collecting and sharing your records. 

How it works

  • You can electronically request and receive records from providers. You can upload information stored on CDs to a Patient Connect account. Doctors can upload diagnostic-quality images and related clinical information into your secure, digital account. 
  • Images and related information are stored securely in one place as a longitudinal record you can easily share with care teams.
