What is MyChart and what can I do with a MyChart account?

MyChart is an online patient portal that gives patients secure access to their health records. It enables patients to securely manage and receive information about their health.

With a verified MyChart account, you can:

  • Schedule, request, reschedule and cancel appointments
  • View upcoming appointments and procedures
  • View test results
  • Request prescription renewals
  • Communicate electronically and securely with your care team
  • View letters and notes created by your healthcare team
  • View and pay bills
  • View your health summary
  • Share portions of your medical record with non-UCI Health providers using Share Everywhere
  • Access trusted health information resources
  • And more!

How is my health information on MyChart protected?

We take great care to ensure your health information is kept private and secure. Access to information is controlled through secure activation codes, personal usernames and passwords. Each person controls their password, and the account cannot be accessed without that password.

Unlike conventional e-mail, all messages between you and your healthcare team are exchanged while you are securely logged in to MyChart. MyChart is fully compliant with federal and state laws pertaining to your privacy.

How do I create a MyChart account?

There are several ways to create a MyChart account.

  • At your next patient visit — Ask your care team how to sign up for MyChart (recommended)
  • Online at my.ucirvinehealth.org — Go to “Sign Up Now”
  • Call MyChart customer support — Call 1-833-4MYCHRT (1-833-469-2478), Monday–Friday from 8 a.m.–5 p.m. PST for help signing up for MyChart

What is an activation code and how do I get one?

Your MyChart activation code is a unique code that allows you to create your username and password after verifying your date of birth and the last four digits of your social security number.

Your activation code is printed on your After Visit Summary (AVS), a document given to you at the end of your outpatient office visit. If you were recently hospitalized at UCI Douglas Hospital, your activation code is printed on your Discharge Summary.

If you misplace your activation code, you can contact UCI Health MyChart Customer Support at 1-833-469-2478. You can choose to receive your activation code by email, text message or United States Postal Service (USPS) mail.

My activation code does not work. What should I do?

For your security, your activation code expires after 30 days or is no longer valid after it has been used. Activation codes sent by text message or email will expire in 24 hours. If you are having issues with your activation code, call MyChart Customer Support at 1-833-469-2478.

Can I view a family member's health information in MyChart?

Yes, proxy access is available in certain situations. Proxy access allows parents/guardians to log in to their personal MyChart account and connect to a family member’s account. If you are interested in obtaining proxy access, complete a Proxy Consent Form and return it to one of our healthcare facilities.

For children ages 0-11, parents/guardians will receive full access to their child’s account. For children ages 12-17, to protect adolescent confidentiality, UCI Health uses a proxy policy compliant with state regulations. Proxies of adolescents are allowed to view certain test results, allergy history, family history and immunizations but cannot view the full health summary, medications, medical history, current health issues, messages and appointments.

Can my spouse and I share one MyChart account?

No. Due to the sensitive nature of medical information, each adult must sign and submit a Release of Information request and establish their own MyChart account.

Can I ask questions regarding a family member from my MyChart account?

No. MyChart offers direct access to your personal health record. Communication from you about another patient would be placed inappropriately in your health record. Any communication in your health record about another patient can jeopardize medical care for you and the other patient.

I was logged out of MyChart. What happened?

UCI Health aims to protect your privacy and the security of your information. If your keyboard remains idle for 15 minutes or more while logged in to MyChart, you will be automatically logged out of MyChart. We recommend that you log out of your MyChart session if you need to leave your computer for even a short period of time.

I previously used the FollowMyHealth patient portal to manage my healthcare with UCI Health. Can I keep my FollowMyHealth account active?

Yes, you can keep your FollowMyHealth account active. You will still have access to your historical health records on FollowMyHealth. However, health information from after Nov. 6, 2018 is added only to MyChart.
