Thank you. 00:00:14:06 - 00:00:15:09 Thank you for having me. 00:00:15:09 - 00:00:16:13 It is lovely to meet you. 00:00:16:13 - 00:00:17:15 And I want to just 00:00:17:15 - 00:00:19:01 I want to come up like straight out 00:00:19:01 - 00:00:19:22 and just front load it 00:00:19:22 - 00:00:20:15 with some really with 00:00:20:15 - 00:00:21:16 some really intense facts 00:00:21:16 - 00:00:23:21 that I didn't know until we sat down. 00:00:23:21 - 00:00:24:21 Heart disease, the number one 00:00:24:21 - 00:00:25:19 killer of men and women 00:00:25:19 - 00:00:26:15 in the United States. 00:00:26:15 - 00:00:27:13 That's right. 00:00:27:13 - 00:00:28:03 Listen to this. 00:00:28:03 - 00:00:30:14 Every 40 seconds someone in the U.S. 00:00:30:14 - 00:00:31:05 has a heart attack. 00:00:31:05 - 00:00:33:07 Every 40 seconds. Every 34 seconds 00:00:33:07 - 00:00:33:22 someone dies of 00:00:33:22 - 00:00:35:15 cardiovascular disease. 00:00:35:15 - 00:00:36:14 I know it affects men 00:00:36:14 - 00:00:37:03 more than women. 00:00:37:03 - 00:00:38:20 We're going to talk about both. 00:00:38:20 - 00:00:39:14 Let's just start with 00:00:39:14 - 00:00:40:15 what are the symptoms 00:00:40:15 - 00:00:42:04 of a heart attack? 00:00:42:04 - 00:00:43:20 Most of the time, classically, 00:00:43:20 - 00:00:45:22 we think of the chest 00:00:45:22 - 00:00:47:13 tightness, chest pressure, 00:00:47:13 - 00:00:49:15 elephant sitting on the chest. 00:00:49:15 - 00:00:50:22 Radiating to the arm. 00:00:50:22 - 00:00:52:19 But oftentimes 00:00:52:19 - 00:00:55:09 that might be a description for men. 00:00:55:09 - 00:00:57:20 But for women it could present 00:00:57:20 - 00:00:59:15 not only as chest pain 00:00:59:15 - 00:01:00:20 but sometimes just jaw 00:01:00:20 - 00:01:03:00 pain or back pain. 00:01:03:00 - 00:01:05:05 Neck fullness. 00:01:05:05 - 00:01:08:11 And these are very atypical symptoms. 00:01:08:13 - 00:01:09:12 The other 00:01:09:12 - 00:01:12:11 non typical symptom is indigestion. 00:01:12:14 - 00:01:14:10 Now typically if you eat 00:01:14:10 - 00:01:15:15 and you have heartburn 00:01:15:15 - 00:01:17:04 30 minutes after a meal, 00:01:17:04 - 00:01:19:16 that's probably real indigestion. 00:01:19:16 - 00:01:21:04 But if you have indigestion 00:01:21:04 - 00:01:22:13 after exertion 00:01:22:13 - 00:01:23:21 that might be a red flag 00:01:23:21 - 00:01:24:23 for a heart disease. 00:01:24:23 - 00:01:25:22 That's really good to know. 00:01:25:22 - 00:01:26:01 Yeah, 00:01:26:01 - 00:01:26:20 and sometimes there's 00:01:26:20 - 00:01:28:03 extreme fatigue. 00:01:28:03 - 00:01:28:22 And nausea 00:01:28:22 - 00:01:29:16 can also be 00:01:29:16 - 00:01:32:02 a presenting symptom for women. Yeah. 00:01:32:02 - 00:01:34:01 Why is that not more widely known? 00:01:34:01 - 00:01:34:21 Because you do. 00:01:34:21 - 00:01:35:08 Like you said, 00:01:35:08 - 00:01:36:06 you only think about, 00:01:36:06 - 00:01:37:06 like, the arm, right? 00:01:37:06 - 00:01:38:10 Like it's just kind. 00:01:38:10 - 00:01:38:20 Yeah. 00:01:38:20 - 00:01:40:09 We discover these things 00:01:40:09 - 00:01:41:20 when we have research 00:01:41:20 - 00:01:44:05 that includes a broad 00:01:44:05 - 00:01:46:04 spectrum of people, including women. 00:01:46:04 - 00:01:47:17 We realize that the symptoms 00:01:47:17 - 00:01:49:08 presentation are different, 00:01:49:08 - 00:01:50:12 but we have to study them 00:01:50:12 - 00:01:52:14 to really pick up on these cues. 00:01:52:14 - 00:01:53:04 Yeah. 00:01:53:04 - 00:01:54:18 Are you finding 00:01:54:18 - 00:01:55:13 that heart disease 00:01:55:13 - 00:01:57:18 is becoming more common in women 00:01:57:18 - 00:01:59:15 then than it was ever before? 00:01:59:15 - 00:02:02:18 Heart disease has been prevalent. 00:02:02:20 - 00:02:03:23 What's different about heart 00:02:03:23 - 00:02:04:21 disease in women 00:02:04:21 - 00:02:07:03 is that we're protected 00:02:07:03 - 00:02:08:15 up until menopause. 00:02:08:15 - 00:02:10:03 After menopause, 00:02:10:03 - 00:02:12:19 we catch up to men in terms of the 00:02:12:19 - 00:02:14:00 amount of heart disease 00:02:14:00 - 00:02:16:02 that affects women as it does in men. 00:02:16:02 - 00:02:17:19 Why? Why is that? 00:02:17:19 - 00:02:18:14 That's fascinating. 00:02:18:14 - 00:02:20:13 Yeah, estrogen is protective. 00:02:20:13 - 00:02:23:05 It keeps our vessels pliable, 00:02:23:05 - 00:02:23:20 keeps good 00:02:23:20 - 00:02:25:00 cholesterol up 00:02:25:00 - 00:02:26:11 and bad cholesterol down. 00:02:26:11 - 00:02:27:06 It keeps blood 00:02:27:06 - 00:02:28:09 glucose in check 00:02:28:09 - 00:02:30:14 and insulin resistance at bay. 00:02:30:14 - 00:02:31:12 But once 00:02:31:12 - 00:02:33:11 perimenopause or menopause 00:02:33:11 - 00:02:35:03 hits, estrogen declines. 00:02:35:03 - 00:02:36:19 And so now we have 00:02:36:19 - 00:02:38:00 a risk of stiffening 00:02:38:00 - 00:02:39:14 of the heart vessels. 00:02:39:14 - 00:02:40:21 We have an increase 00:02:40:21 - 00:02:43:02 in bad cholesterol LDL. 00:02:43:02 - 00:02:43:22 We have a decrease 00:02:43:22 - 00:02:45:13 in the good cholesterol. 00:02:45:13 - 00:02:47:05 There's an increase in belly fat, 00:02:47:05 - 00:02:48:11 which is visceral fat 00:02:48:11 - 00:02:49:07 and visceral fat 00:02:49:07 - 00:02:50:09 has been described 00:02:50:09 - 00:02:51:18 as metabolically active, 00:02:51:18 - 00:02:53:17 which means it's inflammatory. 00:02:53:17 - 00:02:55:01 And that's a risk factor 00:02:55:01 - 00:02:56:03 for heart disease. 00:02:56:03 - 00:02:58:18 Yeah. So, things change. 00:02:58:18 - 00:03:00:12 When estrogen declines. 00:03:00:12 - 00:03:02:10 That is interesting. 00:03:02:10 - 00:03:02:18 Okay. 00:03:02:18 - 00:03:04:02 So what, 00:03:04:02 - 00:03:05:04 what questions should 00:03:05:04 - 00:03:05:23 someone that's 00:03:05:23 - 00:03:06:17 watching this right now 00:03:06:17 - 00:03:07:08 that, you know, 00:03:07:08 - 00:03:08:03 I don't know if it's 00:03:08:03 - 00:03:08:21 runs in the family 00:03:08:21 - 00:03:09:14 or they have concerns. 00:03:09:14 - 00:03:10:05 What questions 00:03:10:05 - 00:03:11:15 do you think are good to start with? 00:03:11:15 - 00:03:13:16 You go to your primary care provider. 00:03:13:16 - 00:03:13:21 Right. 00:03:13:21 - 00:03:14:07 Like what do you 00:03:14:07 - 00:03:15:05 what do we want to know? 00:03:15:05 - 00:03:16:13 Well, I think asking 00:03:16:13 - 00:03:18:05 those questions are so important. 00:03:18:05 - 00:03:21:10 Ask your primary care provider 00:03:21:12 - 00:03:24:07 what is my risk of heart disease? 00:03:24:09 - 00:03:24:21 Am I at 00:03:24:21 - 00:03:25:16 increased risk 00:03:25:16 - 00:03:28:01 because of a family history? 00:03:28:01 - 00:03:30:04 You can ask them, 00:03:30:04 - 00:03:31:06 is there any testing 00:03:31:06 - 00:03:31:17 I could do 00:03:31:17 - 00:03:34:01 to check for early buildup of plaque 00:03:34:01 - 00:03:35:20 or atherosclerosis? 00:03:35:20 - 00:03:37:13 And if you have any symptoms 00:03:37:13 - 00:03:38:20 like shortness of breath 00:03:38:20 - 00:03:40:11 with exertion or chest 00:03:40:11 - 00:03:41:23 tightness, chest pressure, 00:03:41:23 - 00:03:42:19 ask what tests 00:03:42:19 - 00:03:45:14 you can do to get those assessed. 00:03:45:14 - 00:03:47:05 If you smoke, 00:03:47:05 - 00:03:50:05 ask for ways to help you quit. 00:03:50:06 - 00:03:52:07 That's very valuable. 00:03:52:07 - 00:03:53:16 There are a lot of resources 00:03:53:16 - 00:03:54:21 for this. 00:03:54:21 - 00:03:57:00 If you're thinking about conceiving 00:03:57:00 - 00:04:01:04 or pregnancy, we've identified that, 00:04:01:06 - 00:04:04:16 maybe we're not risk stratifying 00:04:04:18 - 00:04:07:10 patients who are becoming pregnant 00:04:07:10 - 00:04:08:17 and they're going into pregnancy 00:04:08:17 - 00:04:10:11 with high risk features. 00:04:10:11 - 00:04:11:13 And ask your doctor, 00:04:11:13 - 00:04:12:22 what are my risks 00:04:12:22 - 00:04:15:14 when it comes to pregnancy and 00:04:15:14 - 00:04:17:16 what can I do to improve them? Yeah. 00:04:17:16 - 00:04:19:07 What are, 00:04:19:07 - 00:04:20:20 the biggest risk factors 00:04:20:20 - 00:04:23:17 for cardiovascular disease? 00:04:23:17 - 00:04:25:17 High blood pressure, 00:04:25:17 - 00:04:27:10 high cholesterol. 00:04:27:10 - 00:04:31:00 Diabetes, smoking and family history. 00:04:31:00 - 00:04:33:02 These are the main five. 00:04:33:02 - 00:04:34:00 There's additional 00:04:34:00 - 00:04:36:04 risk enhancing factors, 00:04:36:04 - 00:04:37:06 but these are the ones 00:04:37:06 - 00:04:39:06 we focus on primarily. 00:04:39:06 - 00:04:42:06 What is, microvascular disease? 00:04:42:06 - 00:04:45:06 Microvascular disease is an area of, 00:04:45:06 - 00:04:46:17 interest of mine. 00:04:46:17 - 00:04:49:09 So there is an entity of angina 00:04:49:09 - 00:04:50:10 or chest pain 00:04:50:10 - 00:04:51:11 that comes from the heart, 00:04:51:11 - 00:04:53:15 but it's not from the typical heart 00:04:53:15 - 00:04:55:08 arteries that form plaque. 00:04:55:08 - 00:04:56:16 These are the tiny little blood 00:04:56:16 - 00:04:57:19 vessels that go inside 00:04:57:19 - 00:04:59:03 the heart muscle. 00:04:59:03 - 00:05:00:15 And when they don't provide 00:05:00:15 - 00:05:01:05 enough blood 00:05:01:05 - 00:05:02:20 flow to the heart muscle, 00:05:02:20 - 00:05:03:19 you get chest 00:05:03:19 - 00:05:05:06 pain and chest tightness, 00:05:05:06 - 00:05:06:00 just like you do 00:05:06:00 - 00:05:07:22 when the big arteries clog. 00:05:07:22 - 00:05:10:12 So we are looking for ways 00:05:10:12 - 00:05:11:21 to identify 00:05:11:21 - 00:05:14:02 this type of heart disease. 00:05:14:02 - 00:05:15:11 And we do that 00:05:15:11 - 00:05:17:11 through specialized testing. 00:05:17:11 - 00:05:19:01 Particularly at UC Irvine, 00:05:19:01 - 00:05:21:00 we do the specialized testing, 00:05:21:00 - 00:05:22:08 but just recognition 00:05:22:08 - 00:05:24:11 that if don't stop at 00:05:24:11 - 00:05:25:22 just looking for blockages 00:05:25:22 - 00:05:27:02 in the big heart vessels, 00:05:27:02 - 00:05:28:13 if you have persistent pain, 00:05:28:13 - 00:05:30:01 and sometimes this pain just goes on 00:05:30:01 - 00:05:31:06 for months or years 00:05:31:06 - 00:05:33:03 and never gets identified, 00:05:33:03 - 00:05:35:01 we want to look for other causes. 00:05:35:01 - 00:05:36:17 Do you find that a lot of, 00:05:36:17 - 00:05:38:08 patients that, you know, end up 00:05:38:08 - 00:05:39:09 having something 00:05:39:09 - 00:05:41:00 wrong, a delay coming 00:05:41:00 - 00:05:42:14 to, to get tested? 00:05:42:14 - 00:05:45:13 In fact? Yes, absolutely. 00:05:45:15 - 00:05:47:04 Women in particular. 00:05:47:04 - 00:05:48:09 Why? 00:05:48:09 - 00:05:51:04 They're looking for the typical chest 00:05:51:04 - 00:05:53:01 pain symptoms that we put out there. 00:05:53:01 - 00:05:53:20 Chest tightness 00:05:53:20 - 00:05:55:12 radiating into the arm. 00:05:55:12 - 00:05:58:20 But with these non-standard 00:05:58:20 - 00:05:59:15 chest pain 00:05:59:15 - 00:06:01:02 or atypical symptoms, 00:06:01:02 - 00:06:03:08 there may be a denial in that 00:06:03:08 - 00:06:03:21 their symptoms 00:06:03:21 - 00:06:05:08 are coming from their heart. 00:06:05:08 - 00:06:06:12 And so there's a delay 00:06:06:12 - 00:06:07:22 in seeking care. 00:06:07:22 - 00:06:09:11 And that is one of the major 00:06:09:11 - 00:06:11:16 delays in the care in women 00:06:11:16 - 00:06:13:15 and also in elderly and diabetics. 00:06:13:15 - 00:06:14:08 They also have 00:06:14:08 - 00:06:16:10 atypical presentations. 00:06:16:10 - 00:06:17:19 But once you hit the door 00:06:17:19 - 00:06:19:05 in that emergency room 00:06:19:05 - 00:06:20:09 and we suspect 00:06:20:09 - 00:06:21:15 you're having a heart attack, 00:06:21:15 - 00:06:22:23 we are working 00:06:22:23 - 00:06:23:14 really hard 00:06:23:14 - 00:06:25:16 to get you to a cardiac cath 00:06:25:16 - 00:06:27:17 lab to do an angiogram and open up 00:06:27:17 - 00:06:29:12 that blood vessel within 90 minutes, 00:06:29:12 - 00:06:31:06 because time matters. 00:06:31:06 - 00:06:32:18 Yeah, and the preserving 00:06:32:18 - 00:06:34:17 heart muscle matters. 00:06:34:19 - 00:06:36:23 You mentioned the testing part of it. 00:06:36:23 - 00:06:38:02 Can you talk about what 00:06:38:02 - 00:06:40:03 that testing entails, 00:06:40:03 - 00:06:40:17 like the testing 00:06:40:17 - 00:06:41:22 you guys do here at UCI health? 00:06:41:22 - 00:06:42:06 Like what? 00:06:42:06 - 00:06:44:09 What is it? What is the test? Sure. 00:06:44:09 - 00:06:47:21 If, we always start with a history, 00:06:47:23 - 00:06:49:03 getting a proper history 00:06:49:03 - 00:06:49:19 and really seeing 00:06:49:19 - 00:06:50:22 if there is elevated 00:06:50:22 - 00:06:52:14 concern for heart blockage 00:06:52:14 - 00:06:53:17 and if there is 00:06:53:17 - 00:06:55:02 a pretty standard test, 00:06:55:02 - 00:06:56:20 is this is a stress test 00:06:56:20 - 00:06:57:11 and that comes in 00:06:57:11 - 00:06:58:16 different modalities, 00:06:58:16 - 00:06:59:23 but the appropriate 00:06:59:23 - 00:07:01:18 stress tests for you. 00:07:01:18 - 00:07:04:00 And of course, if that's abnormal 00:07:04:00 - 00:07:06:02 the next step is the gold standard 00:07:06:02 - 00:07:08:01 which is a cardiac catheterization 00:07:08:01 - 00:07:10:09 to look for a heart blockage. 00:07:10:09 - 00:07:12:09 So let's say we do that test 00:07:12:09 - 00:07:14:08 and there is a heart blockage. 00:07:14:08 - 00:07:16:13 Then we would put a stent, 00:07:16:13 - 00:07:17:22 which opens up the blockage. 00:07:17:22 - 00:07:19:13 But if we're really convinced 00:07:19:13 - 00:07:20:13 you're having 00:07:20:13 - 00:07:21:22 chest pain from the heart 00:07:21:22 - 00:07:23:15 and we don't see a blockage, 00:07:23:15 - 00:07:25:07 then we can move on 00:07:25:07 - 00:07:26:19 to that additional testing 00:07:26:19 - 00:07:27:11 that we do 00:07:27:11 - 00:07:28:07 to look for things 00:07:28:07 - 00:07:30:14 like microvascular disease 00:07:30:14 - 00:07:32:07 or vasal spasm 00:07:32:07 - 00:07:33:15 of the heart arteries. 00:07:33:15 - 00:07:34:08 These are all 00:07:34:10 - 00:07:35:01 causes of 00:07:35:01 - 00:07:36:18 angina or chest pain, 00:07:36:18 - 00:07:39:01 but not due to obstruction. Yeah. 00:07:39:01 - 00:07:40:02 I work with a 00:07:40:02 - 00:07:41:03 there's a guy that works on our show. 00:07:41:03 - 00:07:43:13 He's like a hypochondriac, I think. 00:07:43:13 - 00:07:44:02 I don't know, 00:07:44:02 - 00:07:45:08 but he's always talking about, like, 00:07:45:08 - 00:07:46:15 I feel like I'm shaking and, 00:07:46:15 - 00:07:48:02 you know, it's like, symptom. 00:07:48:02 - 00:07:49:08 And then I go and wait, does my, 00:07:49:08 - 00:07:50:00 you know, it's like. 00:07:50:00 - 00:07:51:04 Like it's a weird thing 00:07:51:04 - 00:07:52:05 sometimes if you get in your head 00:07:52:05 - 00:07:52:15 about it, too, 00:07:52:15 - 00:07:52:20 you're like, 00:07:52:20 - 00:07:53:12 I should go get checked, 00:07:53:12 - 00:07:54:16 but I think everyone should go 00:07:54:16 - 00:07:56:21 get checked if it's a concern. Yes. 00:07:56:21 - 00:07:58:21 I think, 00:07:58:21 - 00:08:02:17 one way I, I advise patients is 00:08:02:19 - 00:08:04:23 if you go for a walk and go 00:08:04:23 - 00:08:06:03 exert yourself 00:08:06:03 - 00:08:07:09 and you feel good 00:08:07:09 - 00:08:08:11 or better, it's 00:08:08:11 - 00:08:10:12 probably not coming from your heart. 00:08:10:12 - 00:08:10:21 Okay? 00:08:10:21 - 00:08:12:18 Because the way we check for heart 00:08:12:18 - 00:08:13:05 related 00:08:13:05 - 00:08:14:03 pain is by 00:08:14:03 - 00:08:14:18 getting your heart 00:08:14:18 - 00:08:17:05 rate up on a treadmill, for example. 00:08:17:05 - 00:08:19:15 So if you can exert yourself 00:08:19:15 - 00:08:22:00 and your pain is improved, 00:08:22:00 - 00:08:23:05 it's probably a good sign. 00:08:23:05 - 00:08:24:21 Okay, so that's a great tip. 00:08:24:21 - 00:08:26:15 That's really fascinating. 00:08:26:15 - 00:08:27:22 What kind of care 00:08:27:22 - 00:08:29:10 do you guys offer here? 00:08:29:10 - 00:08:30:18 At UCI Health? 00:08:30:18 - 00:08:31:07 You know, that 00:08:31:07 - 00:08:32:12 that other that other places 00:08:32:12 - 00:08:34:04 don't or can't, 00:08:34:04 - 00:08:36:10 you know, as an academic center, 00:08:36:10 - 00:08:39:15 we have a wide spectrum of 00:08:39:17 - 00:08:41:16 not only specialists, but programs. 00:08:41:16 - 00:08:43:04 So we can treat 00:08:43:04 - 00:08:45:16 patients from advanced heart failure 00:08:45:16 - 00:08:47:15 to cardio obstetrics, 00:08:47:15 - 00:08:48:15 which is in partnership 00:08:48:15 - 00:08:49:16 with our ob gyn. 00:08:49:16 - 00:08:50:19 So that's very related 00:08:50:19 - 00:08:52:11 to women's heart health 00:08:52:11 - 00:08:54:22 as well as integrative cardiology, 00:08:54:22 - 00:08:56:16 preventive cardiology. 00:08:56:16 - 00:08:57:21 And we partner 00:08:57:21 - 00:08:59:19 that up with our research. 00:08:59:19 - 00:09:01:05 We have cutting edge research. 00:09:01:05 - 00:09:03:08 We have innovation incentives. 00:09:03:08 - 00:09:05:23 We look for the latest tools 00:09:05:23 - 00:09:07:13 and the best treatments 00:09:07:13 - 00:09:08:18 that are out there. 00:09:08:18 - 00:09:09:08 Yeah. 00:09:09:08 - 00:09:11:01 Give to our patients I love that. 00:09:11:01 - 00:09:12:18 Let's talk about prevention. 00:09:12:18 - 00:09:15:11 What can people do to obviously 00:09:15:11 - 00:09:16:09 not smoke. Right. 00:09:16:09 - 00:09:17:09 but what other things can 00:09:17:09 - 00:09:19:06 someone just do day to day, 00:09:19:06 - 00:09:20:11 you know, to 00:09:20:11 - 00:09:21:07 get ahead of it? 00:09:21:10 - 00:09:22:11 The easiest thing 00:09:22:11 - 00:09:24:00 that I've learned is, 00:09:24:00 - 00:09:25:09 making a healthy choice 00:09:25:09 - 00:09:26:09 every time you're faced 00:09:26:09 - 00:09:27:00 with a choice. 00:09:27:00 - 00:09:28:16 Oh, my God. He's become 00:11:47:16 - 00:11:48:15 very passionate, 00:11:48:15 - 00:11:49:03 we'll say, 00:11:49:03 - 00:11:50:07 to make sure that his heart 00:11:50:07 - 00:11:51:18 is not at that same risk. 00:11:51:18 - 00:11:53:20 Gone through so many tests. 00:11:53:20 - 00:11:54:19 But what about somebody 00:11:54:19 - 00:11:56:00 who's not doing all of that? 00:11:56:00 - 00:11:56:23 What are some things to know 00:11:56:23 - 00:11:59:10 about the familial risk? 00:11:59:10 - 00:12:00:08 The definition 00:12:00:08 - 00:12:01:06 for premature 00:12:01:06 - 00:12:02:10 heart disease 00:12:02:10 - 00:12:03:06 in the past 00:12:03:06 - 00:12:04:06 has been considered 00:12:04:06 - 00:12:06:19 any male family member 00:12:06:19 - 00:12:08:05 that had a heart attack 00:12:08:05 - 00:12:10:00 at less than 45 years of age, 00:12:10:00 - 00:12:12:00 or any female family member 00:12:12:00 - 00:12:13:12 first degree relative 00:12:13:12 - 00:12:15:02 that's had a heart attack 00:12:15:02 - 00:12:16:07 under 55 of age. 00:12:16:07 - 00:12:18:23 Okay, so 45, 55, 45, 55 00:12:18:23 - 00:12:21:07 so if you have those, 00:12:21:07 - 00:12:22:22 parameters in your family, 00:12:22:22 - 00:12:25:20 then we recommend early assessment 00:12:25:20 - 00:12:27:01 for heart disease. 00:12:27:01 - 00:12:28:09 That being said, 00:12:28:09 - 00:12:30:13 if there's a first degree relative, 00:12:30:13 - 00:12:32:00 a male, for example, 00:12:32:00 - 00:12:33:13 a brother or a father 00:12:33:13 - 00:12:34:15 that's had a heart attack 00:12:34:15 - 00:12:37:10 at less than 55 years of age, 00:12:37:10 - 00:12:39:19 or a female less than 65 years of 00:12:39:19 - 00:12:42:13 age, I would consider that, 00:12:42:15 - 00:12:43:17 to be on high alert. 00:12:43:17 - 00:12:45:12 Okay, we already know what to do. 00:12:45:12 - 00:12:46:21 So same things, 00:12:46:21 - 00:12:47:16 that you could do 00:12:47:16 - 00:12:48:17 is make sure you've 00:12:48:17 - 00:12:49:17 had your cholesterol 00:12:49:17 - 00:12:51:17 checked, your blood pressure checked, 00:12:51:17 - 00:12:53:17 and there are imaging modalities 00:12:53:17 - 00:12:54:13 that can be done. 00:12:54:13 - 00:12:55:06 For example, 00:12:55:06 - 00:12:57:09 a coronary calcium score has been 00:12:57:09 - 00:13:01:05 well validated to risk stratify 00:13:01:07 - 00:13:03:12 a patient into either low, medium 00:13:03:12 - 00:13:04:08 or high risk. 00:13:04:08 - 00:13:06:05 And this is a CT scan. 00:13:06:05 - 00:13:07:09 It's a very quick CT 00:13:07:09 - 00:13:08:07 scan of the chest 00:13:08:07 - 00:13:10:02 looking for calcium 00:13:10:02 - 00:13:10:23 in the heart arteries, 00:13:10:23 - 00:13:12:07 which is a surrogate 00:13:12:07 - 00:13:14:17 or a marker of heart plaque buildup. 00:13:14:17 - 00:13:16:12 And if you have the presence of that 00:13:16:12 - 00:13:17:16 it essentially just means 00:13:17:16 - 00:13:18:18 that you would treat 00:13:18:18 - 00:13:20:03 your risk factors 00:13:20:03 - 00:13:21:04 more aggressively 00:13:21:04 - 00:13:22:17 and perhaps get on cholesterol 00:13:22:17 - 00:13:25:09 lowering therapies earlier. 00:13:25:09 - 00:13:27:20 And this test has been validated 00:13:27:20 - 00:13:29:18 for those 40 and above. 00:13:29:18 - 00:13:31:14 So if you're 40 and above 00:13:31:14 - 00:13:32:15 and you have a family 00:13:32:15 - 00:13:33:08 history of early 00:13:33:08 - 00:13:34:04 heart disease, 00:13:34:04 - 00:13:35:17 consider discussing that 00:13:35:17 - 00:13:37:13 with your primary care doctor. 00:13:37:13 - 00:13:38:00 Really good. 00:13:38:00 - 00:13:39:07 Thank you so much Doctor Barseghian. 00:13:39:07 - 00:13:40:21 We appreciate the time.
