That's right. 00:00:17:14 - 00:00:18:19 Yes, Did I get it? 00:00:18:19 - 00:00:19:05 Yes, 00:00:19:05 - 00:00:20:18 I also do like abdominal 00:00:20:18 - 00:00:22:07 wall reconstructions, 00:00:22:07 - 00:00:24:01 but I do a lot of different things. 00:00:24:01 - 00:00:25:11 Yeah, but that's the key 00:00:25:11 - 00:00:26:17 to what we're talking about today. 00:00:26:17 - 00:00:27:02 Yeah. 00:00:27:02 - 00:00:28:10 Is that like a good conversation 00:00:28:10 - 00:00:29:02 starter for you. 00:00:29:02 - 00:00:29:23 Like when you're meeting people 00:00:29:23 - 00:00:30:12 like I do 00:00:30:12 - 00:00:32:17 abdominal wall reconstructions? 00:00:32:17 - 00:00:33:11 I start with 00:00:33:11 - 00:00:35:06 I do minimally invasive surgery 00:00:35:06 - 00:00:36:14 that tend to impress 00:00:36:14 - 00:00:38:11 a lot of people that I make. 00:00:38:11 - 00:00:40:10 I do a lot of things with little cuts. 00:00:40:10 - 00:00:41:14 That's so cool. 00:00:41:14 - 00:00:42:15 And as we've been doing this, 00:00:42:15 - 00:00:43:04 that's one of the 00:00:43:04 - 00:00:43:18 I will say 00:00:43:18 - 00:00:44:05 before we 00:00:44:05 - 00:00:45:03 jump into our topic today, 00:00:45:03 - 00:00:45:19 we're going to talk about weight 00:00:45:19 - 00:00:46:12 loss and surgeries 00:00:46:12 - 00:00:48:10 and all that and GLP-1s. 00:00:48:10 - 00:00:51:19 I have been so fascinated to learn 00:00:51:21 - 00:00:54:11 about how little 00:00:54:11 - 00:00:55:20 we need to cut anymore. 00:00:55:20 - 00:00:58:08 We've made remarkable advances. 00:00:58:10 - 00:00:59:20 And basically how we do it 00:00:59:20 - 00:01:01:23 is we use long instruments, 00:01:01:23 - 00:01:03:21 and it actually has even advanced 00:01:03:21 - 00:01:04:10 the surgeries 00:01:04:10 - 00:01:06:06 we are able to offer as a whole. 00:01:06:06 - 00:01:06:12 Yeah. 00:01:06:12 - 00:01:06:18 Like, 00:01:06:18 - 00:01:08:04 if we think about surgeries 00:01:08:04 - 00:01:09:05 deep in the pelvis 00:01:09:05 - 00:01:11:16 or for my part, at surgeries 00:01:11:16 - 00:01:13:08 high up in the chest, 00:01:13:08 - 00:01:14:21 it's a lot easier 00:01:14:21 - 00:01:15:22 with little instruments 00:01:15:22 - 00:01:17:05 because some of those places 00:01:17:05 - 00:01:18:17 you can't even reach with a big cut. 00:01:18:17 - 00:01:19:01 Yeah. 00:01:19:01 - 00:01:22:19 And so so not only is it safer 00:01:22:21 - 00:01:23:19 than before, 00:01:23:19 - 00:01:25:22 but also we're able to do more things. 00:01:25:22 - 00:01:27:14 Sure. Yeah. That's awesome. 00:01:27:14 - 00:01:29:17 Well, let's, let's start with this, 00:01:29:17 - 00:01:31:21 because of of what you do and, 00:01:31:21 - 00:01:32:12 you know, look, 00:01:32:12 - 00:01:33:11 there's a lot of focus on it 00:01:33:11 - 00:01:33:18 right now. 00:01:33:18 - 00:01:35:17 There's a lot of talk and buzz around 00:01:35:17 - 00:01:37:00 GLP-1s and, 00:01:37:00 - 00:01:37:17 you know, feeling 00:01:37:17 - 00:01:38:20 better, looking better. 00:01:38:20 - 00:01:42:00 How should we view obesity? 00:01:42:02 - 00:01:42:12 Yeah. 00:01:42:12 - 00:01:45:03 So this is actually a really 00:01:45:03 - 00:01:46:06 interesting topic. 00:01:46:06 - 00:01:48:22 Even within our field of obesity, 00:01:48:22 - 00:01:50:08 we've been kind of 00:01:50:08 - 00:01:51:11 feeling around in the dark 00:01:51:11 - 00:01:52:06 to figure out a way 00:01:52:06 - 00:01:54:01 that helps patients. The best. 00:01:54:01 - 00:01:55:05 And, 00:01:55:05 - 00:01:56:17 and so now the experts 00:01:56:17 - 00:01:57:16 are starting to come together 00:01:57:16 - 00:01:59:01 reviewing all the data 00:01:59:01 - 00:02:00:07 from all the patients 00:02:00:07 - 00:02:01:01 we've treated over 00:02:01:01 - 00:02:02:12 the last few decades. 00:02:02:12 - 00:02:03:23 And some of the big conclusions 00:02:03:23 - 00:02:04:17 that we're starting 00:02:04:17 - 00:02:05:05 to kind of 00:02:05:05 - 00:02:07:04 emerging concepts is 00:02:07:04 - 00:02:09:00 obesity is a chronic 00:02:09:00 - 00:02:10:23 relapsing and remitting disease, 00:02:10:23 - 00:02:11:13 you know, 00:02:11:13 - 00:02:13:19 so, you know, when we see on TV 00:02:13:19 - 00:02:15:13 they talk about it as yoyo diets. 00:02:15:13 - 00:02:16:04 But what it 00:02:16:04 - 00:02:18:07 really is is a disease 00:02:18:07 - 00:02:20:01 that's treated for a little bit 00:02:20:01 - 00:02:22:14 and then has started coming back. 00:02:22:16 - 00:02:25:01 And so when we view, 00:02:25:01 - 00:02:26:15 obesity as a disease, it's 00:02:26:15 - 00:02:28:10 much easier for me as a doctor 00:02:28:10 - 00:02:29:20 to talk about it with my patients. 00:02:29:20 - 00:02:30:04 Yeah. 00:02:30:04 - 00:02:31:13 Because then the patients 00:02:31:13 - 00:02:32:09 don't feel like, 00:02:32:09 - 00:02:34:17 oh, we're just here for my vanity. 00:02:34:17 - 00:02:36:05 You know, it's a it's a disease 00:02:36:05 - 00:02:37:20 that has consequences 00:02:37:20 - 00:02:40:04 for all your organ systems. 00:02:40:04 - 00:02:40:11 You know, 00:02:40:11 - 00:02:41:22 the ones that my patients feel 00:02:41:22 - 00:02:44:04 first are acid reflux. 00:02:44:04 - 00:02:45:05 They get really bad 00:02:45:05 - 00:02:46:01 joint pain 00:02:46:01 - 00:02:47:10 from carrying all of the weight 00:02:47:10 - 00:02:49:06 all of the time. 00:02:49:06 - 00:02:50:17 They also can have 00:02:50:17 - 00:02:51:21 very high risk of heart 00:02:51:21 - 00:02:53:04 attacks, strokes, 00:02:53:04 - 00:02:55:15 even a lot of different cancers. 00:02:55:15 - 00:02:57:19 And so treating obesity, 00:02:57:19 - 00:02:59:12 I like to think of it as a surgery 00:02:59:12 - 00:03:00:18 that sets you up for long 00:03:00:18 - 00:03:01:10 term success. 00:03:01:10 - 00:03:03:22 Right. And so, 00:03:03:22 - 00:03:06:19 and so that's why we do what we do. 00:03:06:19 - 00:03:10:15 And, and, and I kind of, 00:03:10:17 - 00:03:12:02 encourage patients to 00:03:12:02 - 00:03:13:17 think about it from that lens 00:03:13:17 - 00:03:15:04 so they don't kind of bring that 00:03:15:04 - 00:03:17:00 shame piece about and worried 00:03:17:00 - 00:03:18:12 that it's just for my vanity. 00:03:18:12 - 00:03:20:23 It's really not, is it? 00:03:20:23 - 00:03:22:06 Just like on a personal level, 00:03:22:06 - 00:03:23:12 I guess personal and professional. 00:03:23:12 - 00:03:23:20 Is it 00:03:23:20 - 00:03:25:07 is it difficult for you 00:03:25:07 - 00:03:26:02 to start to have 00:03:26:02 - 00:03:27:02 some of those conversations 00:03:27:02 - 00:03:29:10 with people? It is. Yeah. 00:03:29:10 - 00:03:30:08 And, you know, 00:03:30:08 - 00:03:31:15 I know a couple of years ago 00:03:31:15 - 00:03:34:03 we were talking about, in the media. 00:03:34:03 - 00:03:36:00 We were looking at things 00:03:36:00 - 00:03:39:00 like body positivity and like, 00:03:39:05 - 00:03:41:08 oh, so-and-so is fat shaming 00:03:41:08 - 00:03:42:12 and things like that. 00:03:42:12 - 00:03:45:08 And it makes us not, clinicians, 00:03:45:08 - 00:03:46:01 not just me 00:03:46:01 - 00:03:47:06 and my whole job, 00:03:47:06 - 00:03:48:04 but primary care 00:03:48:04 - 00:03:50:04 doctors, cardiologists. 00:03:50:04 - 00:03:51:00 It's really hard for them 00:03:51:00 - 00:03:52:10 to bring up that topic 00:03:52:10 - 00:03:54:23 without damaging that patient, 00:03:54:23 - 00:03:56:12 patient doctor relationship 00:03:56:12 - 00:03:58:08 that we worked so hard to build. 00:03:58:08 - 00:03:59:15 And so sometimes we say, 00:03:59:15 - 00:03:59:23 you know what? 00:03:59:23 - 00:04:01:02 There's other pressing issues 00:04:01:02 - 00:04:02:03 at this visit. 00:04:02:03 - 00:04:03:17 Let's just put them on the back burner 00:04:03:17 - 00:04:05:04 for now. Right. 00:04:05:04 - 00:04:07:02 But the way I think about it 00:04:07:02 - 00:04:09:12 now is it's an important conversation 00:04:09:12 - 00:04:09:23 to have 00:04:09:23 - 00:04:11:08 so that they can avoid 00:04:11:08 - 00:04:13:03 that open heart surgery down the line. 00:04:13:03 - 00:04:13:20 Yes. 00:04:13:20 - 00:04:16:13 And it's and and and they can avoid 00:04:16:13 - 00:04:19:05 a cancer diagnosis chemo and 00:04:19:05 - 00:04:20:14 and things like that. 00:04:20:14 - 00:04:23:13 And so for me it's I, 00:04:23:13 - 00:04:25:03 I start from a place of 00:04:25:03 - 00:04:27:00 you are when you're in my office, 00:04:27:00 - 00:04:28:16 me and my whole team 00:04:28:16 - 00:04:30:17 of wonderful social workers 00:04:30:17 - 00:04:34:02 and, dietitians, we I foster, 00:04:34:02 - 00:04:35:07 work very hard to foster 00:04:35:07 - 00:04:37:02 a very positive environment. 00:04:37:02 - 00:04:38:10 So our patients feel like 00:04:38:10 - 00:04:40:08 they can talk about anything with us. 00:04:40:08 - 00:04:41:13 But at the same time, 00:04:41:13 - 00:04:42:20 that means that I start 00:04:42:20 - 00:04:43:19 the conversation. 00:04:43:19 - 00:04:44:09 Sometimes they're 00:04:44:09 - 00:04:45:07 not ready to hear it. 00:04:45:07 - 00:04:46:09 Yes, but I 00:04:46:09 - 00:04:48:08 at least start with a question. 00:04:48:08 - 00:04:49:09 And then, 00:04:49:09 - 00:04:50:18 that usually opens up 00:04:50:18 - 00:04:52:09 a lot of things that they were worried 00:04:52:09 - 00:04:53:19 to talk about with me. 00:04:53:19 - 00:04:54:07 And they're like, 00:04:54:07 - 00:04:55:00 oh, it's so good 00:04:55:00 - 00:04:55:18 that you bring that up. 00:04:55:18 - 00:04:58:06 Yeah, I can talk about this with you. 00:04:58:06 - 00:05:00:03 And I feel safe to do so. Yeah. 00:05:00:03 - 00:05:02:22 And so I think if this is something 00:05:02:22 - 00:05:04:15 that's on a patient's mind, 00:05:04:15 - 00:05:06:08 please bring it up with your doctor. 00:05:06:08 - 00:05:07:07 That eliminates 00:05:07:07 - 00:05:10:12 a lot of our fear of, like that 00:05:10:14 - 00:05:13:07 damaging our relationship with you. 00:05:13:07 - 00:05:15:04 And it makes that conversation 00:05:15:04 - 00:05:17:03 so easy on our end. Yeah. 00:05:17:03 - 00:05:18:05 So let's talk about, like, 00:05:18:05 - 00:05:19:21 you know, the the stuff that you do. 00:05:19:21 - 00:05:22:06 Bariatric surgery versus, 00:05:22:06 - 00:05:23:03 you know, like we were saying, 00:05:23:03 - 00:05:24:11 GLP ones are everywhere right now. 00:05:24:11 - 00:05:26:16 Is it a matter of versus to you, 00:05:26:16 - 00:05:28:22 Doctor Wang I don't think so. 00:05:28:22 - 00:05:30:20 I think bottom line is 00:05:30:20 - 00:05:32:04 obesity is a disease, 00:05:32:04 - 00:05:33:16 but the patient 00:05:33:16 - 00:05:35:06 is the captain of the ship, right? 00:05:35:06 - 00:05:36:01 It's your body. 00:05:36:01 - 00:05:37:17 You have the choice to do 00:05:37:17 - 00:05:40:00 treat it however you like. 00:05:40:00 - 00:05:42:04 My job as the doctor at our visit 00:05:42:04 - 00:05:44:03 is to give you all the options, right? 00:05:44:03 - 00:05:45:13 My, my, I have a team 00:05:45:13 - 00:05:47:13 where one of someone on my team 00:05:47:13 - 00:05:49:08 prescribes these medications. 00:05:49:08 - 00:05:51:00 And so I don't see it 00:05:51:00 - 00:05:54:07 as an antagonistic relationship. 00:05:54:09 - 00:05:55:02 Between the two, 00:05:55:02 - 00:05:55:23 I say, 00:05:55:23 - 00:05:57:21 what option fits the best 00:05:57:21 - 00:06:00:21 with you at this moment in your life? 00:06:00:23 - 00:06:02:06 And then we go through the pros 00:06:02:06 - 00:06:04:12 and cons of all of the options. Right. 00:06:04:12 - 00:06:07:23 And so I love bariatric surgery. 00:06:08:01 - 00:06:09:14 Because it's 00:06:09:14 - 00:06:11:09 the only thing that's in the data 00:06:11:09 - 00:06:13:08 that has the most durable long 00:06:13:08 - 00:06:14:19 term success okay. 00:06:14:19 - 00:06:16:12 And so patients we 00:06:16:12 - 00:06:17:00 we now have 00:06:17:00 - 00:06:19:17 can you can you pause one second. Why. 00:06:19:17 - 00:06:20:06 What do you mean by 00:06:20:06 - 00:06:21:22 I go a little deeper on that. Yeah. 00:06:21:22 - 00:06:23:15 So when we talk about success 00:06:23:15 - 00:06:24:06 with weight loss 00:06:24:06 - 00:06:25:16 surgery, the numbers 00:06:25:16 - 00:06:27:16 that we can compare are 00:06:27:16 - 00:06:29:15 how long have you kept the weight off? 00:06:29:15 - 00:06:30:00 Okay. 00:06:30:00 - 00:06:30:17 And so 00:06:30:17 - 00:06:32:15 we've been doing bariatric surgery 00:06:32:15 - 00:06:33:23 for decades. 00:06:33:23 - 00:06:35:23 So we've got long term follow up 00:06:35:23 - 00:06:38:00 data up to like 20 years. 00:06:38:00 - 00:06:39:12 And patients are keeping 00:06:39:12 - 00:06:42:02 that weight off for all of that time. 00:06:42:02 - 00:06:45:13 And the conversely, 00:06:45:13 - 00:06:47:13 the GLP ones have been on the market 00:06:47:13 - 00:06:49:03 for just 2 to 3 years. 00:06:49:03 - 00:06:51:01 They do have really good signal 00:06:51:01 - 00:06:52:22 of like dropping the weight. 00:06:52:22 - 00:06:55:18 But weight loss is not as much as, 00:06:55:18 - 00:06:56:20 bariatric surgery, 00:06:56:20 - 00:06:58:09 but some people really do 00:06:58:09 - 00:07:00:11 very well with these medications. 00:07:00:11 - 00:07:02:06 But what we also know about these meds 00:07:02:06 - 00:07:03:18 is when you stop taking them, 00:07:03:18 - 00:07:04:21 the weight bounces back. 00:07:04:21 - 00:07:05:13 Okay, listen, I'm 00:07:05:13 - 00:07:06:21 going to just be honest with you. 00:07:06:21 - 00:07:08:09 We were even talking about this before 00:07:08:09 - 00:07:09:07 we turned on the cameras. 00:07:09:07 - 00:07:10:19 I have lost 44 pounds in the last. 00:07:10:19 - 00:07:11:17 I don't know, 00:07:11:19 - 00:07:14:03 16, 15, 16 months, whatever. 00:07:14:03 - 00:07:15:07 I notice 00:07:15:07 - 00:07:18:06 if if I don't hit my weekly dose, 00:07:18:06 - 00:07:20:07 I don't even a couple days 00:07:20:07 - 00:07:21:11 later, I'm just like, I. 00:07:21:11 - 00:07:22:15 It comes back. 00:07:22:15 - 00:07:24:03 All I want to do is eat. Yeah. 00:07:24:03 - 00:07:24:15 You know what I mean? 00:07:24:15 - 00:07:26:10 And I can feel it just like that. 00:07:26:10 - 00:07:27:09 So it's you're. 00:07:27:09 - 00:07:28:20 I mean, I'm living proof 00:07:28:20 - 00:07:29:12 that what you just said 00:07:29:12 - 00:07:31:00 is completely accurate. Yeah. 00:07:31:00 - 00:07:33:00 And that weight does come back 00:07:33:00 - 00:07:34:01 if you're not careful. 00:07:34:01 - 00:07:35:06 And so, you know, 00:07:35:06 - 00:07:38:06 I always say that's not kind of, 00:07:38:09 - 00:07:39:09 it's definitely still 00:07:39:09 - 00:07:40:10 the patient's decision 00:07:40:10 - 00:07:41:23 to decide what route 00:07:41:23 - 00:07:43:09 they feel the most comfortable with. 00:07:43:09 - 00:07:43:20 Yeah. 00:07:43:20 - 00:07:44:16 And but like 00:07:44:16 - 00:07:46:19 if they are ready for surgery, 00:07:46:19 - 00:07:48:10 that's the one I encourage patients 00:07:48:10 - 00:07:49:17 to really consider. Okay. 00:07:49:17 - 00:07:51:18 Because that's one thing to do 00:07:51:18 - 00:07:53:00 upfront that 00:07:53:00 - 00:07:53:14 then they're set 00:07:53:14 - 00:07:54:20 for the rest of their life. 00:07:54:20 - 00:07:56:21 What what are the different 00:07:56:21 - 00:07:58:04 I've heard of, 00:07:58:04 - 00:08:00:19 you know, gastric bypass, sleeve. 00:08:00:19 - 00:08:01:05 What are the procedures 00:08:01:05 - 00:08:02:05 What are the procedures 00:08:02:05 - 00:08:02:20 that we're doing 00:08:02:20 - 00:08:03:14 now that you’re doing? 00:08:03:14 - 00:08:05:12 Yeah, those are exactly the two 00:08:05:12 - 00:08:07:05 that are standard of care. 00:08:07:05 - 00:08:09:02 First up, operations that we do. 00:08:09:02 - 00:08:10:07 So the sleeve, it's 00:08:10:07 - 00:08:12:07 the most commonly offered surgery 00:08:12:07 - 00:08:13:13 in the United States. 00:08:13:13 - 00:08:14:14 What we do is, 00:08:14:14 - 00:08:14:22 you know, 00:08:14:22 - 00:08:16:12 I like to think of the 00:08:16:12 - 00:08:18:20 the stomach as, like a purse. 00:08:18:20 - 00:08:19:06 Okay. 00:08:19:06 - 00:08:20:08 That's what it looks like 00:08:20:08 - 00:08:21:19 in the operating. Yes. 00:18:47:22 - 00:18:49:12 You know, a lot of my patients, 00:18:49:12 - 00:18:50:14 one of the biggest things 00:18:50:14 - 00:18:52:01 I actually make my patients come 00:18:52:01 - 00:18:54:00 see me and it's really great, is 00:18:54:00 - 00:18:55:17 my wife's about to have a baby, 00:18:55:17 - 00:18:56:10 and I'm scared 00:18:56:10 - 00:18:57:07 I can't keep up 00:18:57:07 - 00:18:58:14 when they're on the football team 00:18:58:14 - 00:18:59:21 or something like that. Yeah. 00:18:59:21 - 00:19:02:17 And, a lot of my patients, like, 00:19:02:17 - 00:19:04:18 even 3 to 6 months out from surgery, 00:19:04:18 - 00:19:05:12 they're telling me 00:19:05:12 - 00:19:07:00 they're running five K's. 00:19:07:00 - 00:19:09:03 They are keeping up with their kids. 00:19:09:03 - 00:19:11:13 They are so relieved 00:19:11:13 - 00:19:13:16 that their body can function again. 00:19:13:16 - 00:19:15:13 And that, to me, is more 00:19:15:13 - 00:19:17:19 the most important part of the surgery 00:19:17:19 - 00:19:20:03 is that you can live your life again. 00:19:20:03 - 00:19:21:18 That's awesome. Interesting. 00:21:06:01 - 00:21:06:22 And kind of 00:21:06:22 - 00:21:07:23 keep your eye out for stuff. 00:21:07:23 - 00:21:09:22 Yeah, that's cool that you do that. 00:21:09:22 - 00:21:10:20 So those are the benefits. 00:21:10:20 - 00:21:11:19 Let's talk about the risks. 00:21:11:19 - 00:21:12:19 Potential risks? 00:21:12:19 - 00:21:15:00 Yeah, there's the risk... 00:21:15:02 - 00:21:17:20 with surgery, always. 00:21:17:20 - 00:21:19:13 every time we cut the bowel 00:21:19:13 - 00:21:20:07 or the stomach, 00:21:20:07 - 00:21:22:17 there's a risk of leakage. Sure. 00:21:22:17 - 00:21:24:02 Patients with obesity 00:21:24:02 - 00:21:25:13 are at higher risk of things 00:21:25:13 - 00:21:27:02 like blood clots to the lungs 00:21:27:02 - 00:21:29:10 or the legs with surgery. 00:21:29:10 - 00:21:31:04 And there's also, 00:21:31:04 - 00:21:32:11 risk of things 00:21:32:11 - 00:21:35:05 like ulcers in the stomach. 00:21:35:05 - 00:21:36:07 As well as we talked 00:21:36:07 - 00:21:38:08 about the vitamin deficiencies. 00:21:38:08 - 00:21:41:00 And so we do, a lot of things 00:21:41:00 - 00:21:42:01 to kind of minimize 00:21:42:01 - 00:21:44:09 these risk around the time of surgery. 00:21:44:09 - 00:21:46:03 We do very intense diet 00:21:46:03 - 00:21:47:11 changes to minimize 00:21:47:11 - 00:21:48:20 the pressure around where 00:21:48:20 - 00:21:51:03 we've sewn things together. 00:21:51:03 - 00:21:53:15 We also counsel our patients, 00:21:53:15 - 00:21:55:00 about things to look out 00:21:55:00 - 00:21:56:01 for, to call us, 00:21:56:01 - 00:21:57:13 you know, any abdominal pain 00:21:57:13 - 00:21:59:00 after these surgeries? 00:21:59:00 - 00:22:00:06 Not really anticipated. 00:22:00:06 - 00:22:02:07 You need to be calling us. Now. 00:22:51:02 - 00:22:52:12 The old person is now a banana. 00:22:52:12 - 00:22:53:05 Yeah, yeah. 00:22:53:05 - 00:22:54:07 So with that surgery, 00:22:54:07 - 00:22:55:07 you stay in the hospital 00:22:55:07 - 00:22:57:02 for one overnight, okay. Yeah. 00:23:02:13 - 00:23:04:18 And and we're, 00:23:04:18 - 00:23:06:06 I, we usually ask patients 00:23:06:06 - 00:23:07:17 to take about a month off work 00:23:07:17 - 00:23:08:07 so they could really 00:23:08:07 - 00:23:09:21 focus on the diet. 00:23:09:21 - 00:23:10:18 But most people don't 00:23:10:18 - 00:23:11:20 really have a lot of pain 00:23:11:20 - 00:23:13:00 beyond that first week. 00:23:13:00 - 00:23:13:22 No way. Wow. 00:23:13:22 - 00:23:15:13 Yeah. Is the diet when you. 00:23:15:13 - 00:23:15:19 Okay? 00:23:15:19 - 00:23:17:22 So let's say you're home after this. 00:23:17:22 - 00:23:19:08 Is the diet immediately 00:23:19:08 - 00:23:19:19 different, even, 00:23:19:19 - 00:23:21:05 like a liquid diet for a week? Yeah. 00:23:21:05 - 00:23:22:14 So even before surgery, 00:23:22:14 - 00:23:24:10 we do two weeks of a liquid diet. 00:23:24:10 - 00:23:25:13 And that's helping that. 00:23:25:13 - 00:23:26:21 Look at that liver shrink 00:23:26:21 - 00:23:28:09 like we were talking about. 00:23:28:09 - 00:23:29:22 And then that continues for that 00:23:29:22 - 00:23:31:19 first two weeks after surgery. 00:23:31:19 - 00:23:32:22 And then the next two weeks 00:23:32:22 - 00:23:34:21 we do a pureed diet, 00:23:34:21 - 00:23:36:11 and then it's a soft diet. 00:23:36:11 - 00:23:37:03 And then we slowly 00:23:37:03 - 00:23:38:18 go back to regular food 00:23:38:18 - 00:23:40:13 because the stomach is smaller. 00:23:40:13 - 00:23:41:09 You have to eat 00:23:41:09 - 00:23:43:20 smaller meals about six times a day 00:23:43:20 - 00:23:46:03 instead of three square meals. Right. 00:23:46:03 - 00:23:49:00 And then we also very, very strongly 00:23:49:00 - 00:23:49:20 tell patients 00:23:49:20 - 00:23:51:14 to stick to a high protein, 00:23:51:14 - 00:23:53:08 high fiber diet. 00:23:53:08 - 00:23:54:13 The proteins help 00:23:54:13 - 00:23:56:03 you heal from the surgery, 00:23:56:03 - 00:23:58:01 and the fiber and the proteins help 00:23:58:01 - 00:23:59:13 you feel full for longer. 00:23:59:13 - 00:24:00:17 So you're not craving 00:24:00:17 - 00:24:02:04 a lot more foods. 00:24:02:04 - 00:24:03:23 Carbohydrates, especially after 00:24:03:23 - 00:24:05:19 these surgeries, can make patients 00:24:05:19 - 00:24:07:14 not feel as well anymore. 00:24:07:14 - 00:24:08:15 And so we try to stick 00:24:08:15 - 00:24:09:23 to a low carb diet 00:24:09:23 - 00:24:11:04 after the surgery as well. 00:24:11:04 - 00:24:12:19 So that's the banana. Yeah. 00:24:12:19 - 00:24:13:19 What about the egg? 00:24:13:19 - 00:24:15:16 So the only difference with the egg 00:24:15:16 - 00:24:17:06 is you stay one more overnight okay. 00:24:17:06 - 00:24:18:09 Two nights at the hospital. 00:24:18:09 - 00:24:20:00 Nights versus one. Yeah. Yeah. 00:24:20:00 - 00:24:21:14 And then everything else actually 00:24:21:14 - 00:24:24:05 largely feels about the same. Yeah. 00:24:24:05 - 00:24:25:11 And then with the, 00:24:25:11 - 00:24:26:08 with both surgeries, 00:24:26:08 - 00:24:27:21 we want to try to prevent ulcers. 00:24:27:21 - 00:24:29:08 So we tell patients, 00:24:29:08 - 00:24:32:19 you got to quit smoking and alcohol, 00:24:32:21 - 00:24:35:07 and, and even marijuana products 00:24:35:07 - 00:24:37:03 for six months before surgery 00:24:37:03 - 00:24:38:17 and then going into the. 00:24:38:17 - 00:24:40:19 You can never have those things again. 00:24:40:19 - 00:24:42:19 And then if you like to take things 00:24:42:19 - 00:24:45:15 like ibuprofen, Motrin, naproxen, 00:24:45:15 - 00:24:47:02 don't think about stopping that 00:24:47:02 - 00:24:49:07 and switching to maybe a Tylenol 00:24:49:07 - 00:24:51:10 or something else to control the pain. 00:24:51:10 - 00:24:51:18 Also, 00:24:51:18 - 00:24:52:17 knowing that a lot of that 00:24:52:17 - 00:24:54:14 your pain is going to go away 00:24:54:14 - 00:24:56:06 with the weight loss. Yeah. 00:24:56:06 - 00:24:57:18 And so we just got to find 00:24:57:18 - 00:24:58:17 something that works 00:24:58:17 - 00:24:59:11 so that you don't 00:24:59:11 - 00:25:01:18 threaten your surgery, 00:25:01:18 - 00:25:02:02 while you're 00:25:02:02 - 00:25:03:02 just trying to feel better. 00:25:03:02 - 00:25:04:01 That's really important to know. 00:25:04:01 - 00:25:04:15 I didn't know that. 00:25:04:15 - 00:25:06:06 You can't go back to those. 00:25:06:06 - 00:25:06:11 Like, 00:25:06:11 - 00:25:08:07 if someone you know, like to have a 00:25:08:07 - 00:25:10:08 I don't know. Yeah, a nightly drink. 00:25:10:08 - 00:25:10:18 They can't. 00:25:10:18 - 00:25:11:19 They're not doing that. 00:25:11:19 - 00:25:13:16 Yeah. It's it's hard. Yeah. 00:25:13:16 - 00:25:14:08 And you know 00:25:14:08 - 00:25:16:00 we kind of talk about the stresses 00:25:16:00 - 00:25:18:09 that happen after weight loss. 00:25:18:09 - 00:25:19:21 And a lot of times 00:25:19:21 - 00:25:21:02 because food 00:25:21:02 - 00:25:22:09 has been a source of comfort 00:25:22:09 - 00:25:23:21 for a lot of our patients, 00:25:23:21 - 00:25:24:23 when that's taken away 00:25:24:23 - 00:25:26:01 with the surgery, 00:25:26:01 - 00:25:26:18 sometimes people 00:25:26:18 - 00:25:28:13 turn back to their old vices 00:25:28:13 - 00:25:29:08 and then like, 00:25:29:08 - 00:25:30:14 things just snowball 00:25:30:14 - 00:25:31:20 out of control for sure. 00:25:31:20 - 00:25:34:20 So we we stick to a very, very tight, 00:25:34:22 - 00:25:37:02 no no drugs and alcohol situation. 00:25:37:02 - 00:25:40:07 Okay. Yeah. 00:25:40:09 - 00:25:41:18 Does, 00:25:41:18 - 00:25:44:07 does obesity affect fertility. 00:25:44:07 - 00:25:46:00 Yes it does. Yeah. 00:25:46:00 - 00:25:48:23 And so a lot of times, 00:25:48:23 - 00:25:50:00 my patients will tell me, 00:25:50:00 - 00:25:51:01 like I've been trying 00:25:51:01 - 00:25:54:01 to conceive with my, partner 00:25:54:02 - 00:25:55:13 and it's just not happening. 00:25:55:13 - 00:25:57:08 Or the other thing that actually, 00:25:57:08 - 00:25:59:09 I hear about even more commonly 00:25:59:09 - 00:26:00:11 is, women 00:26:00:11 - 00:26:02:17 who come see me and they said, 00:26:02:17 - 00:26:04:07 and they're like, I have obesity. 00:26:04:07 - 00:26:05:00 Let's treat it. 00:26:05:00 - 00:26:08:00 And then I ask them, oh, by the way, 00:26:08:01 - 00:26:10:00 do you have really heavy periods? 00:26:10:00 - 00:26:11:00 And across the board 00:26:11:00 - 00:26:11:15 they're all either 00:26:11:15 - 00:26:13:18 say I really, really heavy periods 00:26:13:18 - 00:26:15:13 or I don't have periods anymore. 00:26:15:13 - 00:26:15:22 And it's 00:26:15:22 - 00:26:16:20 because when 00:26:16:20 - 00:26:18:10 you have an accumulation 00:26:18:10 - 00:26:19:18 of fat in the body, 00:26:19:18 - 00:26:20:12 one of the places 00:26:20:12 - 00:26:23:12 the fat likes to go to is your 00:26:23:12 - 00:26:26:06 pituitary gland in your brain. 00:26:26:06 - 00:26:27:20 And that's where a lot of your, 00:26:27:20 - 00:26:29:18 reproductive hormones are made, 00:26:29:18 - 00:26:31:04 and it gets all out of whack 00:26:31:04 - 00:26:32:18 with obesity. 00:26:32:18 - 00:26:34:11 And so treating the obesity 00:26:34:11 - 00:26:36:14 we do know from earlier 00:26:36:14 - 00:26:38:02 at really studies that are emerging 00:26:38:02 - 00:26:39:03 that we can't 00:26:39:03 - 00:26:39:23 that regulates 00:26:39:23 - 00:26:42:11 your, menses after surgery. 00:26:42:11 - 00:26:44:16 And there's some 00:26:44:18 - 00:26:46:23 some signal in the data that's, 00:26:46:23 - 00:26:48:01 starting to emerge 00:26:48:01 - 00:26:50:21 that a lot of patients can have, 00:26:50:21 - 00:26:53:08 successful pregnancies after surgery. 00:26:53:08 - 00:26:54:17 That being said, 00:26:54:17 - 00:26:56:08 we do not want patients 00:26:56:08 - 00:26:57:00 to get pregnant 00:26:57:00 - 00:26:58:06 within that first year 00:26:58:06 - 00:27:00:08 after bariatric surgery 00:27:00:08 - 00:27:01:17 just because that's the 00:27:01:17 - 00:27:02:03 the year 00:27:02:03 - 00:27:02:13 where you are 00:27:02:13 - 00:27:04:08 going to make the most progress 00:27:04:08 - 00:27:05:06 on your weight loss. 00:27:05:06 - 00:27:06:16 Oh, and so it does pose 00:27:06:16 - 00:27:08:20 a certain risk to the, 00:27:08:20 - 00:27:10:23 the infant as well as the mom.
