Okay. 00:01:09:05 - 00:01:10:15 So a lot of these 00:01:10:15 - 00:01:12:00 unconventional therapies 00:01:12:00 - 00:01:13:19 focus on making the patients 00:01:13:19 - 00:01:15:22 do better from the inside. 00:01:15:22 - 00:01:17:20 Okay, so give me some examples 00:01:17:20 - 00:01:19:18 of when you say outside and inside. 00:01:19:18 - 00:01:20:19 What do you mean? Outside 00:01:20:19 - 00:01:22:06 would be what? Like the chemotherapy? 00:01:22:06 - 00:01:23:08 Yeah. like outside 00:01:23:08 - 00:01:24:08 you got the chemotherapy. 00:01:24:08 - 00:01:25:14 You got the radiation, 00:01:25:14 - 00:01:27:00 the drug infused into your veins. 00:01:27:00 - 00:01:27:15 Sure. 00:01:27:15 - 00:01:30:09 Radiation and surgery. Inside means 00:01:30:09 - 00:01:32:17 you change the internal environment 00:01:32:17 - 00:01:34:21 by changing your diet. 00:01:34:21 - 00:01:36:02 How do you exercise. 00:01:36:02 - 00:01:39:01 How to reduce stress. Sleep. 00:01:39:01 - 00:01:40:04 Circadian rhythm. 00:01:40:04 - 00:01:40:11 Yeah. 00:01:40:11 - 00:01:41:15 Social environment. 00:01:41:15 - 00:01:42:03 Okay. 00:01:42:03 - 00:01:43:09 So these are what we call 00:01:43:09 - 00:01:45:00 changing the ecosystem 00:01:45:00 - 00:01:46:13 the cancer cells are living in. 00:01:46:13 - 00:01:47:02 Okay. 00:01:47:02 - 00:01:48:04 They don't live in isolation. 00:01:48:04 - 00:01:48:22 You the cancer cells 00:01:48:22 - 00:01:50:03 don't live in isolation. 00:01:50:03 - 00:01:51:02 It's in there. 00:01:51:02 - 00:01:52:14 They live in our body. 00:01:52:14 - 00:01:53:05 They interact 00:01:53:05 - 00:01:55:01 with the rest of our body. 00:01:55:01 - 00:01:56:18 For example, what we eat, 00:01:56:18 - 00:01:59:00 the nutrients we eat 00:01:59:02 - 00:01:59:18 expose 00:01:59:18 - 00:02:01:01 every cell in a body, 00:02:01:01 - 00:02:02:23 including cancer cells. 00:02:02:23 - 00:02:05:06 You can make it more supportive 00:02:05:06 - 00:02:06:21 or friendly to cancer cells. 00:02:06:21 - 00:02:09:02 Or you can make it less friendly 00:02:09:02 - 00:02:10:09 or more hostile to cancer cells. 00:02:10:09 - 00:02:12:05 Okay, so this is I've seen this 00:02:12:05 - 00:02:13:14 this phrase the whole person 00:02:13:14 - 00:02:14:14 integrative care right. 00:02:14:14 - 00:02:15:10 The whole person 00:02:15:10 - 00:02:17:05 being the inside of the person. 00:02:17:05 - 00:02:18:17 Yeah. That's what we mean by whole person. 00:02:18:17 - 00:02:20:03 Not just the cancer tissue. 00:02:20:03 - 00:02:20:13 Yeah. 00:02:20:13 - 00:02:23:01 But your cardiovascular system, 00:02:23:01 - 00:02:23:21 your endocrine system. 00:02:23:21 - 00:02:24:20 Your immune system. 00:02:24:20 - 00:02:25:09 Yeah. 00:02:25:09 - 00:02:26:18 Your nerve system. 00:02:26:18 - 00:02:28:16 Because all these are connected 00:02:28:16 - 00:02:29:20 nerve system. 00:02:29:20 - 00:02:31:06 Your emotions change 00:02:31:06 - 00:02:32:20 your endocrine system, 00:02:32:20 - 00:02:34:12 change your reaction 00:02:34:12 - 00:02:36:11 to the biological process 00:02:36:11 - 00:02:38:04 involving cancer. 00:02:38:04 - 00:02:39:02 Your diet can 00:02:39:02 - 00:02:40:16 change the microbiome 00:02:40:16 - 00:02:42:08 or the bacteria living in our gut 00:02:42:08 - 00:02:43:06 that in turn, 00:02:43:06 - 00:02:44:17 change the immune system. 00:02:44:17 - 00:02:45:08 Yes. 00:02:45:08 - 00:02:46:12 So there are some studies 00:02:46:12 - 00:02:47:12 showing for people 00:02:47:12 - 00:02:49:06 who eat a high-fiber diet 00:02:49:06 - 00:02:51:02 or people who take certain 00:02:51:02 - 00:02:52:14 kind of probiotics, 00:02:52:14 - 00:02:54:11 the immunotherapy works better. 00:02:54:11 - 00:02:54:19 Okay. 00:02:54:19 - 00:02:57:02 These are both published on Science 00:02:57:02 - 00:02:58:20 or every top journal. 00:02:58:20 - 00:02:59:02 So it's 00:02:59:02 - 00:03:00:08 kind of like everything either 00:03:00:08 - 00:03:01:15 it's a hand in hand thing. 00:03:01:15 - 00:03:01:21 Yeah. 00:03:01:21 - 00:03:03:01 The more you're taking care 00:03:03:01 - 00:03:03:19 of the inside 00:03:03:19 - 00:03:04:23 the better you're going to respond 00:03:04:23 - 00:03:06:10 to what's coming from the outside. 00:03:06:10 - 00:03:07:00 Exactly. 00:03:07:00 - 00:03:08:14 So this part is, 00:03:08:14 - 00:03:10:09 overlooked in historically. 00:03:10:09 - 00:03:10:21 Right people? 00:03:10:21 - 00:03:11:23 I want to talk about that. Yeah. 00:03:11:23 - 00:03:12:06 Go ahead. 00:03:12:06 - 00:03:12:17 You say that, 00:03:12:17 - 00:03:13:11 and then I have questions 00:03:13:11 - 00:03:14:06 about that part. 00:03:14:06 - 00:03:15:00 Historically, we're 00:03:15:00 - 00:03:17:15 also it's based on our understanding 00:03:17:15 - 00:03:19:01 of how cancer come about. 00:03:19:01 - 00:03:20:06 So initially we thought, 00:03:20:06 - 00:03:20:21 oh, cancer 00:03:20:21 - 00:03:21:17 is because the cells 00:03:21:17 - 00:03:22:16 have the mutation. 00:03:22:16 - 00:03:23:18 The mutations genes. 00:03:23:18 - 00:03:24:22 They grow out of control. 00:03:24:22 - 00:03:26:18 So let's cut the mutation, 00:03:26:18 - 00:03:28:08 kill the cancer cells. Right. 00:03:28:08 - 00:03:29:02 That was great. 00:03:29:02 - 00:03:29:08 Yes. 00:03:29:08 - 00:03:30:15 We made a lot of progress 00:03:30:15 - 00:03:32:20 in targeting cancer cells. 00:03:32:20 - 00:03:35:06 But in the last ten years 00:03:35:06 - 00:03:36:19 or so we found 00:03:36:19 - 00:03:38:04 the supporting environment. 00:03:38:04 - 00:03:39:17 We call it microenvironment. 00:03:39:17 - 00:03:40:07 And bigger 00:03:40:07 - 00:03:41:21 theme is microenvironment. 00:03:41:21 - 00:03:43:10 Yeah. It's also important. 00:03:43:10 - 00:03:43:19 Yeah. 00:03:43:19 - 00:03:45:10 One example is 00:03:45:10 - 00:03:46:01 the cancer 00:03:46:01 - 00:03:47:00 will not grow 00:03:47:00 - 00:03:49:04 beyond the size of a sesame 00:03:49:04 - 00:03:51:20 unless they can recruit blood vessel 00:03:51:20 - 00:03:54:13 to support oxygen and nutrients. 00:03:54:13 - 00:03:55:05 This is a process 00:03:55:05 - 00:03:57:01 called angiogenesis. 00:03:57:01 - 00:03:58:10 So there are drugs being developed 00:03:58:10 - 00:04:00:09 to cut off angiogenesis. 00:04:00:09 - 00:04:02:22 But then they found stress 00:04:02:22 - 00:04:04:23 promote this angiogenesis. Yeah. 00:04:04:23 - 00:04:07:07 So they did some study in mice. 00:04:07:07 - 00:04:08:07 When you stress the mice 00:04:08:07 - 00:04:09:07 two hours a day 00:04:09:07 - 00:04:11:00 the tumor grow fast 00:04:11:00 - 00:04:12:20 because the adrenaline stimulate 00:04:12:20 - 00:04:13:22 the growth of blood vessels 00:04:13:22 - 00:04:15:20 into tumor to support them. 00:04:15:20 - 00:04:18:02 So what we do in the oncology 00:04:18:02 - 00:04:18:23 is to use 00:04:18:23 - 00:04:20:17 a lot of these non drug therapy 00:04:20:17 - 00:04:22:18 to help people reduce stress. 00:04:22:18 - 00:04:23:18 You can not just give people 00:04:23:18 - 00:04:25:14 a prescription to reduce stress. 00:04:25:14 - 00:04:28:08 Like Lorazepam. I wish. Or 00:04:28:08 - 00:04:29:06 Valium. 00:04:29:06 - 00:04:30:15 Sure. It's 00:04:30:15 - 00:04:32:17 it works but it's very narrow. 00:04:32:17 - 00:04:33:12 It doesn't reduce 00:04:33:12 - 00:04:34:10 your overall strategy. 00:04:34:10 - 00:04:35:11 Just make you look like 00:04:35:11 - 00:04:37:05 you are less stressed. Yes. 00:04:37:05 - 00:04:37:14 Okay. 00:04:37:14 - 00:04:39:22 So did you 00:04:39:22 - 00:04:41:06 I don't know whether personally 00:04:41:06 - 00:04:42:08 or you know, 00:04:42:08 - 00:04:44:12 overall, did you have you run into 00:04:44:12 - 00:04:45:09 when you first started. 00:04:45:09 - 00:04:46:01 All of that's right. 00:04:46:01 - 00:04:47:11 Did you run into some 00:04:47:11 - 00:04:49:11 some skeptics or roadblocks 00:04:49:11 - 00:04:49:21 like was this 00:04:49:21 - 00:04:51:04 it was this a tough one for you 00:04:51:04 - 00:04:53:05 to kind of get into? 20 years ago? 00:04:53:05 - 00:04:54:00 That's what I mean. 00:04:54:00 - 00:04:54:19 Now, no. 00:04:54:19 - 00:04:55:08 Right now 00:04:55:08 - 00:04:57:04 it's become widely accepted 00:04:57:04 - 00:04:59:00 that we have to treat cancer 00:04:59:00 - 00:05:00:23 in the context of the whole person. 00:05:00:23 - 00:05:01:14 Absolutely. 00:05:01:14 - 00:05:02:20 What was that turning point? 00:05:02:20 - 00:05:04:02 What what was it 00:05:04:02 - 00:05:05:06 that opened people's eyes? 00:05:05:06 - 00:05:05:14 Is it 00:05:05:14 - 00:05:06:13 the fact that you're 00:05:06:13 - 00:05:07:06 coming at this now 00:05:07:06 - 00:05:08:10 with with evidence, with science, 00:05:08:10 - 00:05:09:21 with studies, with research? 00:05:09:21 - 00:05:10:23 Is that it? 00:05:10:23 - 00:05:12:13 That's the major part, 00:05:12:13 - 00:05:14:08 because a lot of the therapies 00:05:14:08 - 00:05:15:21 we integrate for non-Western 00:05:15:21 - 00:05:16:16 treatment 00:05:16:16 - 00:05:19:00 has been around for hundreds, 00:05:19:00 - 00:05:20:14 if not thousands of years. Right? 00:05:20:14 - 00:05:23:01 I like acupuncture or meditation. 00:05:23:01 - 00:05:23:10 Yeah. 00:05:23:10 - 00:05:25:03 Or how you time 00:05:25:03 - 00:05:27:05 your eating with the 00:05:27:05 - 00:05:29:00 they eat at the time of the day, 00:05:29:00 - 00:05:30:18 you know the circadian rhythm. Yeah. 00:05:30:18 - 00:05:32:04 And so for example, 00:05:32:04 - 00:05:34:22 in traditional Chinese medicine, 00:05:35:00 - 00:05:35:19 certain hours of 00:05:35:19 - 00:05:37:05 day you need to do certain things. 00:05:37:05 - 00:05:38:06 You can have a certain hour. 00:05:38:06 - 00:05:39:15 You go to bed, you eat each meal 00:05:39:15 - 00:05:40:20 a certain time. 00:05:40:20 - 00:05:43:19 So all these were in theory 00:05:43:19 - 00:05:45:00 by tradition. Yes. 00:05:45:00 - 00:05:46:02 What made it 00:05:46:02 - 00:05:48:00 mainstream are these 00:05:48:00 - 00:05:49:11 scientific research. 00:05:49:11 - 00:05:50:13 Most of them happened 00:05:50:13 - 00:05:52:11 in the last ten years or so. 00:05:52:11 - 00:05:53:22 And one example 00:05:53:22 - 00:05:55:19 for the circadian rhythm. Right. 00:05:55:19 - 00:05:57:11 in traditional Chinese medicine 00:05:57:11 - 00:05:59:22 we always say, oh, you should, 00:05:59:22 - 00:06:01:18 you should take a nap around noon 00:06:01:18 - 00:06:02:15 or you should sleep. 00:06:02:15 - 00:06:03:14 You should... don't 00:06:03:14 - 00:06:05:10 go to sleep after midnight. 00:06:05:10 - 00:06:06:21 You go to sleep before midnight. 00:06:06:21 - 00:06:07:10 Right. 00:06:07:10 - 00:06:08:23 So now the people who study 00:06:08:23 - 00:06:10:20 circadian rhythm, 00:06:10:20 - 00:06:13:09 they found every cell in our body 00:06:13:09 - 00:06:15:02 actually have a clock system. 00:06:15:02 - 00:06:16:15 It's called the clock gene system. 00:06:16:15 - 00:06:18:02 Every cell, every cell. 00:06:18:02 - 00:06:18:08 Yeah. 00:06:18:08 - 00:06:19:18 It's universal, 00:06:19:18 - 00:06:22:02 because if the sun goes up and down 00:06:22:02 - 00:06:23:17 and every living organism 00:06:23:17 - 00:06:24:12 is exposed to that. 00:06:24:12 - 00:06:26:05 So yeah, they develop that. 00:06:26:05 - 00:06:26:20 And the 00:06:26:20 - 00:06:27:19 people who studied 00:06:27:19 - 00:06:29:02 and discovered this and studied 00:06:29:02 - 00:06:31:03 this got the Nobel Prize 00:06:31:03 - 00:06:34:11 and I believe in 2017 or 18, 2017. 00:06:34:13 - 00:06:36:16 So that's when people say, “a-ha!” 00:06:36:16 - 00:06:38:00 The circadian rhythm, 00:06:38:00 - 00:06:39:08 the timing 00:06:39:08 - 00:06:40:03 of eating, 00:06:40:03 - 00:06:40:23 sleeping, doing 00:06:40:23 - 00:06:43:05 things are actually important. 00:06:43:05 - 00:06:45:12 So you can see the scientific data 00:06:45:12 - 00:06:47:06 correlate with what 00:06:47:06 - 00:06:48:18 we have been practicing. Yeah. 00:06:48:18 - 00:06:49:13 In some of these 00:06:49:13 - 00:06:51:02 traditional medical systems. 00:06:51:02 - 00:06:53:00 Do you find, 00:06:53:00 - 00:06:54:03 because I would bet 00:06:54:03 - 00:06:55:06 that most people are very bad 00:06:55:06 - 00:06:56:08 at this, 00:06:56:08 - 00:06:59:15 that because of the nature of our 00:06:59:17 - 00:07:00:13 crazy lives, 00:07:00:13 - 00:07:02:09 our jobs our go, go, go, 00:07:02:09 - 00:07:05:03 that most people aren't mindful 00:07:05:03 - 00:07:07:03 of what you're talking about. 00:07:07:03 - 00:07:07:08 Yeah. 00:07:07:08 - 00:07:08:14 So you're 00:07:08:14 - 00:07:09:11 you're cutting right 00:07:09:11 - 00:07:10:09 to the heart of the issue. 00:07:10:09 - 00:07:11:18 We know all these. 00:07:11:18 - 00:07:12:11 But how to change 00:07:12:11 - 00:07:14:04 people's behavior, that's it, 00:07:14:04 - 00:07:16:06 how to make people do better. 00:07:16:06 - 00:07:17:23 The first step is to be mindful. 00:07:17:23 - 00:07:18:08 Yeah. 00:07:18:08 - 00:07:19:10 To be aware 00:07:19:10 - 00:07:21:07 this actually is a thing. 00:07:21:07 - 00:07:23:05 This is actually important. 00:07:23:05 - 00:07:24:14 At UCI 00:07:24:14 - 00:07:25:12 there is a scientist 00:07:25:12 - 00:07:26:21 Dr. Masri. 00:07:26:21 - 00:07:27:19 She studied 00:07:27:19 - 00:07:30:09 this circadian rhythm and timing, 00:07:30:09 - 00:07:33:02 and she found in mice models, 00:07:33:02 - 00:07:34:16 if the mice are sleep deprived 00:07:34:16 - 00:07:35:08 like they sleep 00:07:35:08 - 00:07:36:22 only 2-3 hours a day, 00:07:36:22 - 00:07:37:09 or you're 00:07:37:09 - 00:07:38:20 constantly changing the night-day 00:07:38:20 - 00:07:41:04 cycle. Yeah. Night or nocturnal 00:07:41:04 - 00:07:42:01 animals. 00:07:42:01 - 00:07:45:06 So, their cycle is reverse from us. 00:07:45:08 - 00:07:47:04 If you're constantly change that 00:07:47:04 - 00:07:48:09 the cancer grow faster. 00:07:48:09 - 00:07:49:10 Oh my God. 00:07:49:10 - 00:07:50:19 And immunotherapy 00:07:50:19 - 00:07:52:00 doesn't work as well. 00:07:52:00 - 00:07:54:23 So you can see just by changing 00:07:54:23 - 00:07:55:18 how you do things 00:07:55:18 - 00:07:56:11 every day, 00:07:56:11 - 00:07:59:01 you may create an environment 00:07:59:01 - 00:08:00:00 that's suppresses 00:08:00:00 - 00:08:01:02 tumor growth 00:08:01:02 - 00:08:02:16 and promote immune system 00:08:02:16 - 00:08:04:00 to fight cancer. 00:08:04:00 - 00:08:05:16 We are trying to research it 00:08:05:16 - 00:08:07:10 in human to move 00:08:07:10 - 00:08:09:16 what she found in mice 00:08:09:16 - 00:08:12:04 and convert it translate to human. 00:08:12:04 - 00:08:13:05 This way we can develop 00:08:13:05 - 00:08:15:00 better cancer treatment. Yeah. 00:08:15:00 - 00:08:16:12 So you mentioned, 00:08:16:12 - 00:08:18:15 how every cell has a clock. 00:08:18:15 - 00:08:21:02 And pardon my ignorance on this. 00:08:21:02 - 00:08:21:21 Is my clock 00:08:21:21 - 00:08:22:20 different than your clock, 00:08:22:20 - 00:08:24:06 or is it relatively the same? 00:08:24:06 - 00:08:25:15 Most people are the same. 00:08:25:15 - 00:08:27:07 But then you got the night owls 00:08:27:07 - 00:08:29:01 whose clock is different. 00:08:29:01 - 00:08:30:02 But each one of us 00:08:30:02 - 00:08:31:18 have our own rhythm. 00:08:31:18 - 00:08:32:22 So my advice you already. 00:08:32:22 - 00:08:33:15 To our patients, 00:08:33:15 - 00:08:35:03 the first step is 00:08:35:03 - 00:08:37:11 just don't keep changing day by day, 00:08:37:11 - 00:08:37:23 okay? 00:08:37:23 - 00:08:38:17 If you're 00:08:38:17 - 00:08:41:12 if you're a night person, fine. 00:08:41:12 - 00:08:42:16 Just do that. 00:08:42:16 - 00:08:43:19 But it's not the best 00:08:43:19 - 00:08:44:22 for most people, 00:08:44:22 - 00:08:46:02 because our natural rhythm 00:08:46:02 - 00:08:47:15 actually goes by the sun. 00:08:47:15 - 00:08:48:04 Yeah. 00:08:48:04 - 00:08:51:14 Imagine... 5000 years ago. 00:08:51:14 - 00:08:53:08 The candles are expensive. 00:08:53:08 - 00:08:54:12 There's no light. 00:08:54:12 - 00:08:55:20 So people would just go by the sun. 00:08:55:20 - 00:08:56:11 The sun goes up. 00:08:56:11 - 00:08:57:18 You start to be active. 00:08:57:18 - 00:08:59:01 When the sun goes down. 00:08:59:01 - 00:09:00:23 That's it. You go to rest. 00:09:00:23 - 00:09:03:02 So most people have that rhythm. 00:09:03:02 - 00:09:03:23 Of course, occasionally 00:09:03:23 - 00:09:04:22 you have some genetic. 00:09:04:22 - 00:09:06:05 There are people who are genetically 00:09:06:05 - 00:09:08:01 programed to do it differently. 00:09:08:01 - 00:09:08:21 That's the problem 00:09:08:21 - 00:09:10:04 with our modern life. 00:09:10:04 - 00:09:12:00 Yeah. And so on. 00:22:23:17 - 00:22:24:13 So the second line 00:22:24:13 - 00:22:25:19 research is outcome. 00:22:25:19 - 00:22:27:16 So we're trying to implement 00:22:27:16 - 00:22:30:01 this whole person health index 00:22:30:01 - 00:22:31:11 to patients. 00:22:31:11 - 00:22:32:22 This is a nine question 00:22:32:22 - 00:22:34:10 asking you about the things 00:22:34:10 - 00:22:35:17 I just mentioned 00:22:35:17 - 00:22:36:16 like diet exercise 00:22:36:16 - 00:22:38:15 stress social and so on. 00:22:38:15 - 00:22:41:10 And we want to collect this data 00:22:41:10 - 00:22:42:05 and track them 00:22:42:05 - 00:22:44:06 throughout their cancer course 00:22:44:06 - 00:22:46:10 and see whether what we're doing 00:22:46:10 - 00:22:47:23 improve their health 00:22:47:23 - 00:22:49:10 that the whole person level, 00:22:49:10 - 00:22:50:13 not just, oh, 00:22:50:13 - 00:22:52:01 are we reducing your pain? 00:22:52:01 - 00:22:53:02 We actually look at 00:22:53:02 - 00:22:54:06 the nine dimension 00:22:54:06 - 00:22:55:08 of your whole person health. 00:22:55:08 - 00:22:55:18 Yeah. 00:22:55:18 - 00:22:58:02 So, so that'll be a direction 00:22:58:02 - 00:23:01:01 of the research.
