Our pleasure. 00:00:30:28 - 00:00:32:09 So 00:00:32:09 - 00:00:33:24 as somebody who is kind of 00:00:33:24 - 00:00:35:14 trying to figure out, like, 00:00:35:14 - 00:00:36:29 am I in perimenopause, 00:00:36:29 - 00:00:38:11 what is perimenopause? 00:00:38:11 - 00:00:39:27 Do I know what perimenopause is? 00:00:39:27 - 00:00:41:15 We just we have a lot to cover. 00:00:41:15 - 00:00:43:29 So what is menopause? 00:00:43:29 - 00:00:45:20 Let's just start from the very beginning. 00:00:45:20 - 00:00:46:18 What is it? 00:00:46:18 - 00:00:47:27 What are the stages? 00:00:47:27 - 00:00:50:09 What age do people usually go into 00:00:50:09 - 00:00:50:24 menopause? 00:00:50:24 - 00:00:52:27 Is there an age we're all waiting for? 00:00:52:27 - 00:00:54:05 Does it happen for different people 00:00:54:05 - 00:00:55:10 at different times? 00:00:55:10 - 00:00:56:11 Tell us everything. 00:00:56:11 - 00:00:57:22 I want to preface menopause 00:00:57:22 - 00:00:59:16 and perimenopause with that 00:00:59:16 - 00:00:59:24 it is 00:00:59:24 - 00:01:00:18 a largely 00:01:00:18 - 00:01:02:04 very individualized 00:01:02:04 - 00:01:03:22 experience for people. 00:01:03:22 - 00:01:05:23 But when we look at average definitions, 00:01:05:23 - 00:01:07:03 because I think that helps set 00:01:07:03 - 00:01:08:00 the stage. 00:01:08:00 - 00:01:08:24 Menopause, 00:01:08:24 - 00:01:09:29 at least from the medicine 00:01:09:29 - 00:01:11:00 background, is defined 00:01:11:00 - 00:01:13:06 as 12 months without a period. 00:01:13:06 - 00:01:14:15 And this is naturally. So 00:01:14:15 - 00:01:15:14 if you've had a surgery 00:01:15:14 - 00:01:17:00 that removed your uterus 00:01:17:00 - 00:01:17:23 or treatments 00:01:17:23 - 00:01:18:29 that stopped your periods, 00:01:18:29 - 00:01:21:06 intentionally or unintentionally, 00:01:21:06 - 00:01:21:23 it's a little bit 00:01:21:23 - 00:01:23:06 of a different definition. 00:01:23:06 - 00:01:25:00 But in general, for the average 00:01:25:00 - 00:01:26:09 individual, it's 12 months 00:01:26:09 - 00:01:27:15 without a period. 00:01:27:15 - 00:01:29:27 Perimenopause precedes this. 00:01:29:27 - 00:01:31:08 And this can be interpreted 00:01:31:08 - 00:01:32:18 also in a lot of ways. 00:01:32:18 - 00:01:34:20 Yeah. This is a very gray, area isn't it? 00:01:34:20 - 00:01:35:10 Absolutely. 00:01:35:10 - 00:01:36:10 I'm still trying to figure out 00:01:36:10 - 00:01:37:05 where I'm at with this. 00:01:37:05 - 00:01:38:09 There's a lot of ways to 00:01:38:09 - 00:01:39:11 look at it, honestly. 00:01:39:11 - 00:01:41:15 In the whole research area, 00:01:41:15 - 00:01:43:11 they have tried to standardize it 00:01:43:11 - 00:01:45:18 to make it easier for comparison. Okay. 00:01:45:18 - 00:01:47:29 We have a lot more studies in menopause 00:01:47:29 - 00:01:49:12 than in perimenopause. 00:01:49:12 - 00:01:51:12 So trying to figure out how we can 00:01:51:12 - 00:01:51:29 make it 00:01:51:29 - 00:01:53:24 equal and relatable 00:01:53:24 - 00:01:56:00 and comparable is important. 00:01:56:00 - 00:01:57:27 So a lot of perimenopause 00:01:57:27 - 00:01:59:11 from the research perspective 00:01:59:11 - 00:02:01:17 looks at your timing of periods. Okay. 00:02:01:17 - 00:02:02:28 So once you start 00:02:02:28 - 00:02:03:26 having some mild 00:02:03:26 - 00:02:06:04 irregularities in periods, variations 00:02:06:04 - 00:02:07:28 about a week 00:02:07:28 - 00:02:09:18 early, a week late 00:02:09:18 - 00:02:12:02 could be early perimenopause. Okay. 00:02:12:02 - 00:02:13:10 And is a lot of that 00:02:13:10 - 00:02:14:18 sort of fine-tuned 00:02:14:18 - 00:02:16:04 and figured out through blood work. 00:02:16:04 - 00:02:18:03 Or is it how? 00:02:18:03 - 00:02:20:20 How much can we nail down perimenopause? 00:02:20:20 - 00:02:21:29 It's a really good question. 00:02:21:29 - 00:02:23:04 So unfortunately, 00:02:23:04 - 00:02:24:21 unlike a lot of medical conditions, 00:02:24:21 - 00:02:26:10 blood work is not very reliable 00:02:26:10 - 00:02:27:27 for perimenopause. 00:02:27:27 - 00:02:29:12 We can use it to help us 00:02:29:12 - 00:02:30:20 sometimes in those scenarios 00:02:30:20 - 00:02:33:00 where we can't rely on menstrual cycles. 00:02:33:00 - 00:02:34:16 But I would consider menstrual cycles 00:02:34:16 - 00:02:35:18 as a pretty important 00:02:35:18 - 00:02:36:22 vital sign 00:02:36:22 - 00:02:38:05 in terms of knowing where you are 00:02:38:05 - 00:02:40:24 in the reproductive axis. Okay. 00:02:40:24 - 00:02:42:14 But blood work can give us sometimes 00:02:42:14 - 00:02:43:03 a clue. 00:02:43:03 - 00:02:44:03 But the problem is, 00:02:44:03 - 00:02:45:24 when you look at the reference ranges 00:02:45:24 - 00:02:47:05 like that little sidebar 00:02:47:05 - 00:02:49:04 that has what is normal. Oh. 00:02:49:04 - 00:02:51:08 It's like 50 to 500. 00:02:51:08 - 00:02:52:21 And it's like wild. 00:02:52:21 - 00:02:53:12 And... Yeah. 00:02:53:12 - 00:02:54:22 If it's in a normal range, 00:02:54:22 - 00:02:56:06 it doesn't necessarily mean 00:02:56:06 - 00:02:57:20 that you're not in perimenopause. Right. 00:02:57:20 - 00:02:59:00 Or that it's normal for you. 00:02:59:00 - 00:03:00:06 Right, exactly. 00:03:00:06 - 00:03:01:04 So unfortunately, 00:03:01:04 - 00:03:02:08 we don't have anything 00:03:02:08 - 00:03:03:23 that we can reliably use 00:03:03:23 - 00:03:06:10 as a one-time test to confirm it. 00:03:06:10 - 00:03:06:29 Okay. 00:03:06:29 - 00:03:09:27 So let's talk about the basics. 00:03:09:27 - 00:03:11:12 Let's say somebody’s coming in 00:03:11:12 - 00:03:12:28 and they don't know what's happening 00:03:12:28 - 00:03:13:22 to them. 00:03:13:22 - 00:03:15:16 What causes menopause? 00:03:15:16 - 00:03:16:05 So when we 00:03:16:05 - 00:03:18:23 look at the reproductive function 00:03:18:23 - 00:03:20:04 there's a lot of communication 00:03:20:04 - 00:03:22:16 from your brain to the ovary. Okay. 00:03:22:16 - 00:03:24:12 The brain secretes a hormone called 00:03:24:12 - 00:03:26:17 follicle-stimulating hormone. Okay. 00:03:26:17 - 00:03:28:08 And it's trying to get your ovaries 00:03:28:08 - 00:03:30:28 to spit out an egg or ovulate. 00:03:30:28 - 00:03:33:02 So when that brain communicates 00:03:33:02 - 00:03:33:20 to the ovary, 00:03:33:20 - 00:03:34:21 your estrogen levels 00:03:34:21 - 00:03:37:05 increase until you ovulate. Okay. 00:03:37:05 - 00:03:38:11 As we start to approach 00:03:38:11 - 00:03:39:24 perimenopause and menopause, 00:03:39:24 - 00:03:41:27 our ovaries are running out of eggs. 00:03:41:27 - 00:03:43:29 So our brain is working harder and harder 00:03:43:29 - 00:03:45:10 to try to get that same level 00:03:45:10 - 00:03:47:11 of estrogen response that it was before, 00:03:47:11 - 00:03:49:16 when we were in our 20s and 30s. 00:03:49:16 - 00:03:51:15 And eventually your ovaries are 00:03:51:15 - 00:03:53:03 no longer ovulating. 00:03:53:03 - 00:03:55:02 So the brain is very hyperactive. 00:03:55:02 - 00:03:55:16 And that's why 00:03:55:16 - 00:03:57:21 we get a lot of central brain symptoms. 00:03:57:21 - 00:04:00:09 It's because a lot of the overactivity 00:04:00:09 - 00:04:02:04 of the pituitary gland 00:04:02:04 - 00:04:04:16 trying to get your ovaries to ovulate. 00:04:04:16 - 00:04:05:10 And eventually 00:04:05:10 - 00:04:07:11 the ovaries is what secretes estrogen, 00:04:07:11 - 00:04:09:06 and that will eventually decline. 00:04:09:06 - 00:04:11:02 And once that ceases to function, 00:04:11:02 - 00:04:12:10 we're in menopause. Okay. 00:04:12:10 - 00:04:13:23 So like the book says, 00:04:13:23 - 00:04:15:00 it starts with an egg. 00:04:15:00 - 00:04:17:11 And so the egg sort of controls 00:04:17:11 - 00:04:18:24 how much estrogen is released 00:04:18:24 - 00:04:20:23 and how it's released. 00:04:20:23 - 00:04:21:11 Okay. 00:04:21:11 - 00:04:22:21 And roughly 00:04:22:21 - 00:04:25:03 I mean, you don't, you know, within ten. 00:04:25:03 - 00:04:25:17 I'm kidding. 00:04:25:17 - 00:04:26:03 But roughly 00:04:26:03 - 00:04:27:24 how many eggs are we born with? 00:04:27:24 - 00:04:30:11 Oh, like millions. Millions. Yes. 00:04:30:11 - 00:04:31:09 We have the most eggs 00:04:31:09 - 00:04:33:03 when we're actually infants 00:04:33:03 - 00:04:34:08 in our mothers’s womb. Right. 00:04:34:08 - 00:04:36:02 And it starts to decline. 00:04:36:02 - 00:04:37:11 And then once we're in puberty, 00:04:37:11 - 00:04:38:02 then we start to 00:04:38:02 - 00:04:39:20 release the eggs more regularly, 00:04:39:20 - 00:04:41:20 and then it starts to accelerate. 00:04:41:20 - 00:04:43:17 Right. After, in our mid 30s 00:04:43:17 - 00:04:45:29 to 40s, until on the average age of 50 00:04:45:29 - 00:04:47:10 got it. There’s no more. Okay. 00:04:47:10 - 00:04:49:08 So when it comes to 00:04:49:08 - 00:04:50:17 all this stuff, there's, you know, 00:04:50:17 - 00:04:51:20 the synthetic hormones, 00:04:51:20 - 00:04:52:16 there's the bioidenticals, 00:04:52:16 - 00:04:54:26 there's all kinds of things we can do. 00:04:54:26 - 00:04:57:04 What about food? 00:04:57:04 - 00:04:58:21 We hear all this stuff 00:04:58:21 - 00:04:59:27 about what we can control, 00:04:59:27 - 00:05:00:24 which we know is not much, 00:05:00:24 - 00:05:02:28 but we can control our diet. 00:05:02:28 - 00:05:03:29 We can control our food. 00:05:03:29 - 00:05:06:11 And we are getting inundated 00:05:06:11 - 00:05:07:12 with protein talk. 00:05:07:12 - 00:05:08:08 I feel like 00:05:08:08 - 00:05:09:21 the cycle has come around again 00:05:09:21 - 00:05:10:28 where everything's protein. 00:05:10:28 - 00:05:13:08 Take your weight. Divide it by two. 00:05:13:08 - 00:05:14:28 Multiply it by seven. 00:05:14:28 - 00:05:16:04 That's how much protein you’re supposed to have 00:05:16:04 - 00:05:16:20 before noon. 00:05:16:20 - 00:05:17:29 Like how 00:05:17:29 - 00:05:19:28 important or unimportant is protein? 00:05:19:28 - 00:05:21:16 It's a really good question. 00:05:21:16 - 00:05:22:00 We do 00:05:22:00 - 00:05:23:03 see a lot of metabolic 00:05:23:03 - 00:05:24:21 changes in midlife, 00:05:24:21 - 00:05:26:15 and we lose muscle mass 00:05:26:15 - 00:05:27:05 more rapidly, 00:05:27:05 - 00:05:28:05 especially as we enter 00:05:28:05 - 00:05:29:03 the time that most of us 00:05:29:03 - 00:05:31:11 are entering perimenopause and menopause. 00:05:31:11 - 00:05:32:17 There's also changes 00:05:32:17 - 00:05:35:03 where we get more insulin resistant too 00:05:35:03 - 00:05:37:09 so there can be sugar changes as well. 00:05:37:09 - 00:05:38:04 So to kind of 00:05:38:04 - 00:05:39:26 help stabilize that or optimize 00:05:39:26 - 00:05:41:26 it, honestly, a well-balanced diet 00:05:41:26 - 00:05:43:02 is the most important. 00:05:43:02 - 00:05:46:00 That incorporates protein but also fiber. 00:05:46:00 - 00:05:47:24 And I want to think like just longevity. 00:05:47:24 - 00:05:49:02 I'm a little bit cautious 00:05:49:02 - 00:05:50:03 about these really high 00:05:50:03 - 00:05:51:10 protein requirements 00:05:51:10 - 00:05:52:14 that are the sole thing 00:05:52:14 - 00:05:53:14 that perimenopause 00:05:53:14 - 00:05:55:02 and midlife women need. 00:05:55:02 - 00:05:56:18 Some of these numbers that we see 00:05:56:18 - 00:05:58:05 are kind of like extrapolated 00:05:58:05 - 00:05:59:09 from what we use in 00:05:59:09 - 00:06:01:03 critically ill patients in the hospital 00:06:01:03 - 00:06:03:05 and their protein needs, which 00:06:03:05 - 00:06:04:17 in certain medical scenarios 00:06:04:17 - 00:06:05:21 may be applicable, 00:06:05:21 - 00:06:06:23 but probably not 00:06:06:23 - 00:06:08:18 for the average perimenopausal patient. 00:06:08:18 - 00:06:09:05 That doesn't mean 00:06:09:05 - 00:06:11:00 that I need to go on a carnivore diet 00:06:11:00 - 00:06:11:21 tomorrow. 00:06:11:21 - 00:06:12:00 No. 00:06:12:00 - 00:06:12:23 No, honestly, 00:06:12:23 - 00:06:14:11 and from a longevity standpoint, 00:06:14:11 - 00:06:15:16 we want to think well balanced. 00:06:15:16 - 00:06:17:03 Like make sure you're getting your fiber, 00:06:17:03 - 00:06:17:28 make sure you're getting 00:06:17:28 - 00:06:19:02 your macronutrients 00:06:19:02 - 00:06:20:18 and all the other kind of vitamins 00:06:20:18 - 00:06:21:21 and micronutrients 00:06:21:21 - 00:06:23:21 that come in all sorts of food. Okay. 00:06:23:21 - 00:06:25:08 So I think that overall 00:06:25:08 - 00:06:27:20 is the safest option. 00:06:27:20 - 00:06:29:04 I think, 00:06:29:04 - 00:06:30:24 we don't have enough information 00:06:30:24 - 00:06:32:00 about the long-term effects 00:06:32:00 - 00:06:32:29 of very high 00:06:32:29 - 00:06:36:04 protein diets alone for perimenopause 00:06:36:04 - 00:06:37:15 in terms of its health impacts. 00:06:37:15 - 00:06:39:15 So incorporate it. Don't live by it. 00:06:39:15 - 00:06:40:22 Or as a nutritionist 00:06:40:22 - 00:06:42:06 friend of mine used to say to me, 00:06:42:06 - 00:06:43:06 stay on the perimeter 00:06:43:06 - 00:06:44:18 of the grocery store. 00:06:44:18 - 00:06:45:17 Don't go through the aisles 00:06:45:17 - 00:06:46:16 of all the process stuff. 00:06:46:16 - 00:06:47:10 Stay at the deli. 00:06:47:10 - 00:06:48:08 Stay at the butcher shop. 00:06:48:08 - 00:06:49:05 Stay in the produce. 00:06:49:05 - 00:06:50:12 That's right. That's okay. 00:06:50:12 - 00:06:51:11 So that makes sense. 00:06:51:11 - 00:06:53:15 Now, I was told a while ago 00:06:53:15 - 00:06:55:12 and I never really-double checked it. 00:06:55:12 - 00:06:56:14 I just stopped eating it. 00:06:56:14 - 00:06:57:11 I love 00:06:57:11 - 00:06:58:27 edamame, and I used to put 00:06:58:27 - 00:07:00:00 soy milk in my coffee, 00:07:00:00 - 00:07:02:05 but I am kind of off of soy 00:07:02:05 - 00:07:02:24 because I 00:07:02:24 - 00:07:04:18 was told that it's estrogen heavy 00:07:04:18 - 00:07:05:24 and it can mess with women. 00:07:05:24 - 00:07:06:22 Is that true? 00:07:06:22 - 00:07:08:28 The estrogen activity of soy 00:07:08:28 - 00:07:11:00 is very, very weak. 00:07:11:00 - 00:07:11:23 So I think when you look 00:07:11:23 - 00:07:13:16 at the whole context of everything, 00:07:13:16 - 00:07:14:03 the hormone 00:07:14:03 - 00:07:15:21 circulating in your own bloodstream 00:07:15:21 - 00:07:17:03 are going to be much more potent 00:07:17:03 - 00:07:18:09 than what some soy milk 00:07:18:09 - 00:07:20:14 or edamame can add to. 00:07:20:14 - 00:07:21:08 So overall, 00:07:21:08 - 00:07:22:09 I think there's a lot more health 00:07:22:09 - 00:07:24:03 benefits in those foods as being, 00:07:24:03 - 00:07:25:04 you know, 00:07:25:04 - 00:07:27:04 offering a lot of micronutrients 00:07:27:04 - 00:07:28:17 and fiber and, 00:07:28:17 - 00:07:29:24 you know, protein 00:07:29:24 - 00:07:31:09 and other other sources of, 00:07:31:09 - 00:07:33:00 you know, nutrients. Yeah. 00:07:33:00 - 00:07:34:14 And then the risk of estrogen 00:07:34:14 - 00:07:36:17 is extremely low. Okay. 00:07:36:17 - 00:07:39:10 So even if I'm ordering like two garlic 00:07:39:10 - 00:07:40:16 edamames for myself 00:07:40:16 - 00:07:42:12 and like the shells are up to here, 00:07:42:12 - 00:07:44:20 I'm not doing real harm to my body. 00:07:44:20 - 00:07:45:15 No. Enjoy it. 00:07:45:15 - 00:07:46:28 It's probably better for your health. 00:07:46:28 - 00:07:47:22 To do that. Good. 00:07:47:22 - 00:07:49:18 I have a plan for tonight. Okay. 00:07:49:18 - 00:07:51:11 So that really helps 00:07:51:11 - 00:07:52:05 people understand, 00:07:52:05 - 00:07:53:16 because, like you said, 00:07:53:16 - 00:07:54:29 there's so much misinformation 00:07:54:29 - 00:07:57:12 and we don't want to do it wrong. 00:07:57:12 - 00:07:58:26 So by telling us, 00:07:58:26 - 00:08:00:20 you know, everything in moderation, 00:08:00:20 - 00:08:02:00 keep it well balanced. 00:08:02:00 - 00:08:04:03 Don't be afraid just for being afraid’s 00:08:04:03 - 00:08:05:17 sake is very helpful. 00:08:05:17 - 00:08:08:12 But when it comes to being cautious 00:08:08:12 - 00:08:09:11 and we talk about like, 00:08:09:11 - 00:08:10:21 third-party services, 00:08:11:21 - 00:08:14:21 I'm curious what is... 00:08:15:08 - 00:08:15:15 I mean, 00:08:15:15 - 00:08:16:15 I guess everything's allowed 00:08:16:15 - 00:08:17:10 if you know the right person, 00:08:17:10 - 00:08:18:21 but what is allowed? 00:08:18:21 - 00:08:19:22 And people even talk 00:08:19:22 - 00:08:21:18 about testosterone therapy, 00:08:21:18 - 00:08:22:22 and you wouldn't think that 00:08:22:22 - 00:08:23:29 a woman needs testosterone, 00:08:23:29 - 00:08:25:11 but that's probably 00:08:25:11 - 00:08:26:26 important to a degree. 00:08:26:26 - 00:08:28:04 So can you just talk a little bit 00:08:28:04 - 00:08:29:11 about both of those things? 00:08:29:11 - 00:08:29:22 Yeah. 00:08:29:22 - 00:08:30:00 You know, 00:08:30:00 - 00:08:30:16 I think there's 00:08:30:16 - 00:08:31:24 a lot of ways to look at it. 00:08:31:24 - 00:08:34:04 I always preface that I trained, 00:08:34:04 - 00:08:34:29 I have an MD, 00:08:34:29 - 00:08:36:22 so I trained in allopathic medicine. 00:08:36:22 - 00:08:37:28 So this is my bias 00:08:37:28 - 00:08:39:20 and this is my background. Sweet! I’m ready. 00:08:39:20 - 00:08:40:18 And there's a lot... Right? 00:08:40:18 - 00:08:42:14 There's a lot of ways to approach health. 00:08:42:14 - 00:08:43:14 And I think there's 00:08:43:14 - 00:08:44:21 a growing body of people 00:08:44:21 - 00:08:46:12 going into more functional 00:08:46:12 - 00:08:48:29 or naturopathic and integrative medicine. 00:08:48:29 - 00:08:49:29 And these can oftentimes 00:08:49:29 - 00:08:51:06 collaborate with each other. 00:08:51:06 - 00:08:53:10 But also there are some differences 00:08:53:10 - 00:08:54:00 and variations 00:08:54:00 - 00:08:54:14 in the way 00:08:54:14 - 00:08:56:05 that we approach things 00:08:56:05 - 00:08:57:14 like midlife care. 00:08:57:14 - 00:08:59:09 So in my perspective, 00:08:59:09 - 00:09:01:04 you know, I think finding someone who, 00:09:01:04 - 00:09:01:10 you know, 00:09:01:10 - 00:09:02:02 I think it helps 00:09:02:02 - 00:09:03:03 when people are certified 00:09:03:03 - 00:09:03:14 from like 00:09:03:14 - 00:09:04:23 the Menopause Society. Sure. Again, 00:09:04:23 - 00:09:05:21 not necessary 00:09:05:21 - 00:09:07:09 to be a midlife provider. Okay. 00:18:48:21 - 00:18:49:10 But I have women 00:18:49:10 - 00:18:52:10 menstruating all the way up to nearly 60. 00:18:52:23 - 00:18:54:04 And then also women 00:18:54:04 - 00:18:55:24 who go into menopause in their 40s. 00:18:55:24 - 00:18:57:10 So there's a broad range of still 00:18:57:10 - 00:18:59:12 what can be considered normal. Absolutely. 00:23:05:21 - 00:23:08:10 So let's talk a little bit about HRT, 00:23:08:10 - 00:23:09:28 about hormone replacement therapy. 00:23:09:28 - 00:23:12:12 Now, for some people it's discouraged. 00:23:12:12 - 00:23:14:00 It has been discouraged in recent years. 00:23:14:00 - 00:23:15:08 And that's sort of it's 00:23:15:08 - 00:23:16:21 reentering the conversation now. 00:23:16:21 - 00:23:19:02 And like I said, it's sort of, 00:23:19:02 - 00:23:20:22 for lack of a better word, trendy. 00:23:20:22 - 00:23:21:23 It's popular 00:23:21:23 - 00:23:22:23 for women, 00:23:22:23 - 00:23:23:03 you know, 00:23:23:03 - 00:23:26:02 in their mid 40s to start asking 00:23:26:02 - 00:23:27:14 their gynecologist, like, 00:23:27:14 - 00:23:28:20 so what about HRT? 00:23:28:20 - 00:23:29:28 Like that's just it's, 00:23:29:28 - 00:23:31:10 it's we're we're 00:23:31:10 - 00:23:32:24 sort of fishing in the dark here 00:23:32:24 - 00:23:33:08 going, okay, 00:23:33:08 - 00:23:33:22 that's something 00:23:33:22 - 00:23:34:17 I'm supposed to ask about. 00:23:34:17 - 00:23:36:00 Well, where are we with that? 00:23:36:00 - 00:23:37:09 So why 00:23:37:09 - 00:23:39:21 why would that even be controversial? 00:23:39:21 - 00:23:41:08 So I think it all stems from 00:23:41:08 - 00:23:41:16 I mean, 00:23:41:16 - 00:23:43:02 if you look at the history of hormone 00:23:43:02 - 00:23:44:29 replacement therapy, initially, 00:23:44:29 - 00:23:47:14 I think one of the concepts was 00:23:47:14 - 00:23:47:28 we noticed 00:23:47:28 - 00:23:49:12 women were having heart attacks 00:23:49:12 - 00:23:50:10 after menopause. 00:23:50:10 - 00:23:51:12 So what if we just give 00:23:51:12 - 00:23:52:03 the hormones back? 00:23:52:03 - 00:23:52:29 Is that going to protect 00:23:52:29 - 00:23:54:03 their heart health? 00:23:54:03 - 00:23:55:02 And then when you started it, 00:23:55:02 - 00:23:56:02 women felt great. 00:23:56:02 - 00:23:57:26 They were like, wow, I feel amazing. 00:23:57:26 - 00:23:59:12 So the prescription 00:23:59:12 - 00:24:00:27 for hormone therapy exploded. 00:24:00:27 - 00:24:01:27 And then they started to do 00:24:01:27 - 00:24:02:29 more formalized studies. 00:24:02:29 - 00:24:04:23 And there was a large study in the 90s, 00:24:04:23 - 00:24:06:10 the Women's Health Initiative, 00:24:06:10 - 00:24:08:15 that was stopped early 00:24:08:15 - 00:24:10:02 because there was an unacceptably high 00:24:10:02 - 00:24:11:17 risk of breast cancer. 00:24:11:17 - 00:24:11:29 And again, 00:24:11:29 - 00:24:13:27 it wasn't the absolute... It wasn’t the risk 00:24:13:27 - 00:24:14:18 you're going to get breast 00:24:14:18 - 00:24:15:27 cancer more often than not, 00:24:15:27 - 00:24:17:10 it was higher than what 00:24:17:10 - 00:24:18:26 we expected, really. 00:24:18:26 - 00:24:19:24 So it was stopped. 00:24:19:24 - 00:24:21:18 And that was the most publicized part of 00:24:21:18 - 00:24:22:11 that study. 00:24:22:11 - 00:24:23:17 So I don't want breast cancer. 00:24:23:17 - 00:24:25:04 I don't want HRT. And, 00:36:53:24 - 00:36:54:14 I think 00:36:54:14 - 00:36:55:21 just to carve out 00:36:55:21 - 00:36:56:14 time for yourself, 00:36:56:14 - 00:36:57:21 make yourself important. 00:36:57:21 - 00:36:58:10 Remember that 00:36:58:10 - 00:37:00:27 if you are not functioning well, the... 00:37:00:27 - 00:37:02:00 society... Yes. 00:38:14:20 - 00:38:15:24 But at the same time, 00:38:15:24 - 00:38:17:06 I've heard it both ways. 00:38:17:06 - 00:38:18:23 I think, you know, men 00:38:18:23 - 00:38:20:21 can be obviously very compassionate 00:38:20:21 - 00:38:21:24 and very understanding 00:38:21:24 - 00:38:23:02 and some of the most understanding 00:38:23:02 - 00:38:24:00 healthcare providers. 00:38:24:00 - 00:38:25:14 And sometimes there are also women 00:38:25:14 - 00:38:26:14 providers who are like, you know, 00:38:26:14 - 00:38:26:29 I did it. 00:38:26:29 - 00:38:28:16 I, it's not a big deal. Suck it up. 00:38:28:16 - 00:38:29:16 You're going to be fine, 00:38:29:16 - 00:38:31:16 which is also not appropriate either. 00:38:31:16 - 00:38:33:03 So I've seen it both ways.
