That's a big resume. 00:00:32:20 - 00:00:34:08 It's a lot you got going on. 00:00:34:08 - 00:00:35:17 Let's first of all talk about this, 00:00:35:17 - 00:00:36:11 and I'm coming 00:00:36:11 - 00:00:37:23 in, like, super naive here. 00:00:37:23 - 00:00:39:00 I did not know there was a 00:00:39:00 - 00:00:41:02 Wilderness Medicine Fellowship program. 00:00:41:02 - 00:00:41:13 I don't know that 00:00:41:13 - 00:00:42:13 a lot of people know that. 00:00:42:13 - 00:00:43:15 Yeah, yeah, there are 00:00:43:15 - 00:00:44:18 Wilderness Medicine Fellowship 00:00:44:18 - 00:00:46:10 programs throughout the country. 00:00:46:10 - 00:00:47:15 And they're available 00:00:47:15 - 00:00:50:00 to many different types of doctors. 00:00:50:00 - 00:00:51:04 Obviously, we would assume 00:00:51:04 - 00:00:53:04 that emergency physicians are 00:00:53:04 - 00:00:53:23 likely a good fit 00:00:53:23 - 00:00:54:21 for wilderness medicine, 00:00:54:21 - 00:00:56:06 but certainly other types of doctors 00:00:56:06 - 00:00:57:07 are involved as well. 00:00:57:07 - 00:00:58:10 So what is your before 00:00:58:10 - 00:01:00:06 we jump into some of the stuff 00:01:00:06 - 00:01:01:10 the meat and potatoes here. 00:01:01:10 - 00:01:02:16 What does a day look like for you? 00:01:02:16 - 00:01:04:12 What's your day to day like? 00:01:04:12 - 00:01:06:05 For me... lots of meetings. 00:01:06:05 - 00:01:07:17 So lots of non-exciting stuff. 00:01:07:17 - 00:01:08:06 Okay. 00:01:08:06 - 00:01:09:18 As you expect, the administration. 00:01:09:18 - 00:01:10:10 Yeah. 00:01:10:10 - 00:01:10:21 But, you know, 00:01:10:21 - 00:01:13:03 may have a shift in the ER. 00:01:13:03 - 00:01:13:19 Okay, 00:01:13:19 - 00:01:14:18 maybe working 00:01:14:18 - 00:01:16:19 with the fire department 00:01:16:19 - 00:01:18:00 several years. 00:01:18:00 - 00:01:18:17 Having days 00:01:18:17 - 00:01:19:15 where I'm working in the field 00:01:19:15 - 00:01:21:17 with their rescue unit. Yeah. 00:01:21:17 - 00:01:24:09 At times deployed to disaster zones 00:01:24:09 - 00:01:25:12 for the federal government, 00:01:25:12 - 00:01:26:15 which I don't do anymore. 00:01:26:15 - 00:01:28:03 But previously, 00:01:28:03 - 00:01:29:04 so a smattering of 00:01:29:04 - 00:01:30:22 of different things. Definitely. 00:01:30:22 - 00:01:32:04 Emergency medicine is a career 00:01:32:04 - 00:01:33:03 where you have to, 00:01:33:03 - 00:01:33:23 you know, wake up 00:01:33:23 - 00:01:35:14 and look at your calendar for the day 00:01:35:14 - 00:01:37:15 because it can be vastly different. 00:01:37:15 - 00:01:39:08 What is emergency medicine like? 00:01:39:08 - 00:01:40:20 What falls under that? What is that? 00:01:40:20 - 00:01:41:04 Yeah. 00:01:41:04 - 00:01:43:07 So emergency medicine 00:01:43:07 - 00:01:45:10 was told by me before medical school 00:01:45:10 - 00:01:46:17 by an emergency physician 00:01:46:17 - 00:01:48:01 that managing anything 00:01:48:01 - 00:01:49:09 they can get carried through the door 00:01:49:09 - 00:01:51:03 for about two hours. Okay. 00:01:51:03 - 00:01:53:21 So a very broad set of diseases 00:01:53:21 - 00:01:54:20 and injuries. 00:01:54:20 - 00:01:57:02 You have to assess. Yeah. 00:01:57:02 - 00:01:58:15 Make sure you don't send anything home 00:01:58:15 - 00:02:00:10 that needs emergency treatment 00:02:00:10 - 00:02:02:10 and start that appropriate treatment 00:02:02:10 - 00:02:03:22 or arrange for the follow up 00:02:03:22 - 00:02:05:13 if the patient's safe for discharge. 00:02:05:13 - 00:02:06:12 That's emergency medicine 00:02:06:12 - 00:02:07:02 in a nutshell. 00:02:07:02 - 00:02:07:14 That's cool. 00:02:07:14 - 00:02:10:18 So, obviously every year I think 00:02:10:18 - 00:02:12:11 the first snowfall or two we, 00:02:12:11 - 00:02:14:09 we see on the news 00:02:14:09 - 00:02:16:00 and we hear stories of people, 00:02:16:00 - 00:02:16:12 you know, 00:02:16:12 - 00:02:17:20 maybe not doing so well 00:02:17:20 - 00:02:18:22 up in the mountains on a hike, 00:02:18:22 - 00:02:19:12 getting stranded, 00:02:19:12 - 00:02:20:11 getting lost, whatever. 00:02:20:11 - 00:02:21:03 Or is that something 00:02:21:03 - 00:02:22:21 that you deal with? Yeah. 00:02:22:21 - 00:02:24:09 You know, for many years, 00:02:24:09 - 00:02:26:06 I worked as a flight medic, 00:02:26:06 - 00:02:28:01 air rescue unit in our area. 00:02:28:01 - 00:02:30:02 And, so that unit, a unit 00:02:30:02 - 00:02:31:06 would periodically involved 00:02:31:06 - 00:02:32:14 in those in those missions. 00:02:32:14 - 00:02:33:17 Yeah, for sure. 00:02:33:17 - 00:02:36:22 What makes the, I guess our mountains, 00:02:36:22 - 00:02:37:16 because we do have, 00:02:37:16 - 00:02:39:11 you know, quite a vast 00:02:39:11 - 00:02:40:11 topography here. 00:02:40:11 - 00:02:42:15 What makes them more difficult, 00:02:42:15 - 00:02:44:04 you know, and more dangerous, 00:02:44:04 - 00:02:46:03 I guess, at this time of year. Yeah. 00:02:46:03 - 00:02:48:20 I think this area is so unique. 00:02:48:20 - 00:02:49:08 And, yeah, 00:02:49:08 - 00:02:50:15 we have the best weather, 00:02:50:15 - 00:02:50:22 you know, 00:02:50:22 - 00:02:51:23 go toe to toe with anyone 00:02:51:23 - 00:02:52:21 who wants to hike. 00:02:52:21 - 00:02:54:02 Otherwise, 00:02:54:02 - 00:02:55:23 but despite having the best weather, 00:02:55:23 - 00:02:56:06 you know, 00:02:56:06 - 00:02:57:07 we have these mountains, 00:02:57:07 - 00:02:59:08 especially Baldy and San Gorgonio, 00:02:59:08 - 00:03:01:04 that are nearby. 00:03:01:04 - 00:03:02:03 And they have 00:03:02:03 - 00:03:03:13 pretty significant elevation, 00:03:03:13 - 00:03:04:13 given how close we are 00:03:04:13 - 00:03:05:21 to the to the ocean 00:03:05:21 - 00:03:07:23 and how close we are to major cities. 00:03:07:23 - 00:03:09:06 And so 00:03:09:06 - 00:03:11:04 it's very easy 00:03:11:04 - 00:03:12:21 from a distance standpoint, 00:03:12:21 - 00:03:16:18 from someone to go from 70 degrees to 00:03:16:18 - 00:03:19:03 refrozen snow and ice. 00:03:19:05 - 00:03:20:12 And perhaps 00:03:20:12 - 00:03:21:05 easier here than 00:03:21:05 - 00:03:23:13 a lot of places in the world. 00:03:23:13 - 00:03:26:01 And I think that does add 00:03:26:01 - 00:03:26:21 an element of danger 00:03:26:21 - 00:03:29:03 that's more easily accessible 00:03:29:03 - 00:03:30:18 than in other areas of the world. 00:03:30:18 - 00:03:32:19 Did you call it Refrozen ice? 00:03:32:19 - 00:03:33:21 Is that what you call it? 00:03:33:21 - 00:03:34:20 Yeah. So, so yeah. 00:03:34:20 - 00:03:35:19 Snow that's melted 00:03:35:19 - 00:03:37:02 and then frozen over. 00:03:37:02 - 00:03:39:09 So is that more dangerous? 00:03:39:09 - 00:03:39:17 Yeah. 00:03:39:17 - 00:03:41:09 I pick snow over ice any day. 00:03:41:09 - 00:03:42:17 It's less slippery. 00:03:42:17 - 00:03:45:06 But now... see, I'm like the don't 00:03:45:06 - 00:03:46:08 don't put me anywhere near that. 00:03:46:08 - 00:03:47:15 I don't I'm fine on the ground. 00:03:47:15 - 00:03:48:11 Is there a bigger problem 00:03:48:11 - 00:03:50:00 when the ice is covered by snow? 00:03:50:00 - 00:03:51:08 You don't know what's under it. 00:03:51:08 - 00:03:52:03 Is that a problem? Yeah. 00:03:52:03 - 00:03:53:19 So refreezing, 00:03:53:19 - 00:03:56:01 layering, all these are issues 00:03:56:01 - 00:03:58:00 for traction and avalanches 00:03:58:00 - 00:03:59:12 and all those inherent dangers. 00:03:59:12 - 00:04:00:17 Like, I grew up in the Midwest. 00:04:00:17 - 00:04:01:11 I think you said 00:04:01:11 - 00:04:02:11 you were from the East Coast, right? 00:04:02:11 - 00:04:03:04 Yeah. So, yeah. 00:04:03:04 - 00:04:06:01 So I know that even to driving a car, 00:04:06:01 - 00:04:07:09 you tend to not know 00:04:07:09 - 00:04:08:05 what's under the snow. 00:04:08:05 - 00:04:08:20 If it's snowing, 00:04:08:20 - 00:04:09:16 you don't know if there's black 00:04:09:16 - 00:04:10:09 ice under there. Right. 00:04:10:09 - 00:04:11:11 So there's an element of danger 00:04:11:11 - 00:04:12:19 just even driving. 00:04:12:19 - 00:04:14:02 Yeah. For sure. 00:04:14:02 - 00:04:15:21 And... but you do know 00:04:15:21 - 00:04:16:20 the road underneath 00:04:16:20 - 00:04:18:03 it is relatively flat 00:04:18:03 - 00:04:20:01 based on construction standards. 00:04:20:01 - 00:04:21:18 And that assurance 00:04:21:18 - 00:04:22:21 doesn't exist in the mountains. 00:04:22:21 - 00:04:23:13 I guess not. 00:04:23:13 - 00:04:25:13 What's the problem I guess? 00:04:25:13 - 00:04:27:12 Do you find that people, 00:04:27:12 - 00:04:28:07 maybe bite off 00:04:28:07 - 00:04:29:18 more than they can chew, so to speak? 00:04:29:18 - 00:04:30:17 In getting out there, 00:04:30:17 - 00:04:32:12 they don't even understand the risks. 00:04:32:12 - 00:04:33:04 I could, 00:04:33:04 - 00:04:33:22 you know, lay out 00:04:33:22 - 00:04:35:06 what some of the possible things are. 00:04:35:06 - 00:04:35:13 Yeah. 00:04:35:13 - 00:04:37:18 The mountains are like the ocean. 00:04:37:18 - 00:04:39:18 You could go on the ocean on a day 00:04:39:18 - 00:04:41:20 and there is very little danger. 00:04:41:20 - 00:04:43:16 You're on a boat, there's no waves. 00:04:43:16 - 00:04:45:02 Everything's wonderful. 00:04:45:02 - 00:04:46:10 And you could go back there 00:04:46:10 - 00:04:47:10 a week later 00:04:47:10 - 00:04:48:19 and be in a life 00:04:48:19 - 00:04:50:14 threatening environment. 00:04:50:14 - 00:04:51:16 And the mountains are similar 00:04:51:16 - 00:04:52:15 to that with weather, 00:04:52:15 - 00:04:54:05 especially if you compare 00:04:54:05 - 00:04:56:06 hiking in the summer, 00:04:56:08 - 00:04:56:21 to the 00:04:56:21 - 00:04:59:07 same mountain in the winter 00:04:59:07 - 00:05:00:23 after snow and ice. Yeah. 00:05:00:23 - 00:05:02:10 And at that point, 00:05:02:10 - 00:05:04:03 I wouldn't even call it hiking. 00:05:04:03 - 00:05:05:23 I would call it now mountaineering. 00:05:05:23 - 00:05:08:10 So same place, same location, 00:05:08:10 - 00:05:10:02 different activity based on weather. 00:05:10:02 - 00:05:12:18 And I think that's the big change. 00:05:12:18 - 00:05:14:22 Is there an element 00:05:14:22 - 00:05:16:10 of maybe bravado 00:05:16:10 - 00:05:18:00 that gets in people's way. 00:05:18:00 - 00:05:18:23 They get in their own way 00:05:18:23 - 00:05:19:07 where it's like 00:05:19:07 - 00:05:20:00 I know this mountain, 00:05:20:00 - 00:05:20:17 I've done this before. 00:05:20:17 - 00:05:21:14 But they did it in the summer 00:05:21:14 - 00:05:21:21 and now it's 00:05:21:21 - 00:05:23:06 an entirely different thing. 00:05:23:06 - 00:05:24:16 Yeah, I could certainly speculate 00:05:24:16 - 00:05:25:15 that someone says, hey, 00:05:25:15 - 00:05:26:20 I've done that before. Yeah. 00:05:26:20 - 00:05:27:16 Just like, 00:05:27:16 - 00:05:28:08 you know, people 00:05:28:08 - 00:05:29:01 go to the beach 00:05:29:01 - 00:05:30:05 and they can swim 00:05:30:05 - 00:05:32:03 when there's no rip current 00:05:32:03 - 00:05:33:23 and there's low swell. 00:05:33:23 - 00:05:35:05 They go to the same beach 00:05:35:05 - 00:05:36:20 and maybe feel confident 00:05:36:20 - 00:05:38:05 based on that experience 00:05:38:05 - 00:05:40:00 and ends up being a very 00:05:40:00 - 00:05:42:01 dangerous decision. 00:05:42:03 - 00:05:43:16 I certainly think the fact 00:05:43:16 - 00:05:45:09 that these mountains 00:05:45:09 - 00:05:48:07 are so close to the city also. 00:05:48:07 - 00:05:49:20 Yes, 00:05:49:20 - 00:05:50:19 may make 00:05:50:19 - 00:05:52:04 people feel a little more comfortable, 00:05:52:04 - 00:05:54:14 like it's a false sense of security. 00:05:54:14 - 00:05:56:03 We're not on Everest. Yeah. 00:05:56:03 - 00:05:57:10 I can't speak for people 00:05:57:10 - 00:05:58:16 that have had issues of course. 00:05:58:16 - 00:05:59:07 But you know, 00:05:59:07 - 00:06:00:08 we could certainly speculate 00:06:00:08 - 00:06:02:03 that could be a problem. Yeah. 00:06:02:03 - 00:06:02:17 So okay, 00:06:02:17 - 00:06:04:11 let's say someone's going to go go, 00:06:04:11 - 00:06:05:13 you know 00:06:05:13 - 00:06:06:19 mountaineer on one of these things. 00:06:06:19 - 00:06:08:02 What would you recommend 00:06:08:02 - 00:06:08:18 people take 00:06:08:18 - 00:06:10:02 before they start this thing. 00:06:10:02 - 00:06:11:02 What special equipment 00:06:11:02 - 00:06:12:17 or set of skills. Yeah. 00:06:12:17 - 00:06:15:17 So skills are, a big part of this. 00:06:15:20 - 00:06:16:15 And I'm certainly 00:06:16:15 - 00:06:17:19 not a knowledgeable 00:06:17:19 - 00:06:19:20 mountaineer myself. 00:06:19:20 - 00:06:21:11 But I think skills 00:06:21:11 - 00:06:23:01 and using the skills 00:06:23:01 - 00:06:23:18 with the equipment. 00:06:23:18 - 00:06:26:01 So what are the big equipment items? 00:06:26:01 - 00:06:27:18 Anywhere where you are 00:06:27:18 - 00:06:29:21 far from civilization, 00:06:29:21 - 00:06:33:05 and you may need emergency assistance. 00:06:33:07 - 00:06:33:21 Mountain. 00:06:33:21 - 00:06:36:02 No mountain snow, no snow. 00:06:36:02 - 00:06:38:12 Having a GPS beacon, 00:06:38:12 - 00:06:40:03 whether that be a PLB 00:06:40:03 - 00:06:43:09 personal locator beacon or another 00:06:43:09 - 00:06:45:06 two-way satellite messenger 00:06:45:06 - 00:06:47:15 that allows you to request help, 00:06:47:17 - 00:06:49:10 or one of the newer phones 00:06:49:10 - 00:06:51:02 that is equipped 00:06:51:02 - 00:06:53:12 with satellite capabilities. 00:06:53:12 - 00:06:54:08 That's a must, 00:06:54:08 - 00:06:55:04 because 00:06:55:04 - 00:06:59:03 when you are out in the wilderness, 00:06:59:05 - 00:07:00:02 a lot of times 00:07:00:02 - 00:07:01:12 there is no cell signal. Right? 00:07:01:12 - 00:07:03:03 And we live all day. All night. 00:07:03:03 - 00:07:03:20 We assume our cell 00:07:03:20 - 00:07:04:13 phones are going to work 00:07:04:13 - 00:07:05:03 and they're going to solve 00:07:05:03 - 00:07:05:20 all of problems. 00:07:05:20 - 00:07:07:21 Yes, until there's no signal 00:07:07:21 - 00:07:09:02 and there's no battery. Yeah. 00:07:09:02 - 00:07:10:14 And oh, by the way, 00:07:10:14 - 00:07:12:01 when there's no signal, 00:07:12:01 - 00:07:13:00 your battery works 00:07:13:00 - 00:07:14:14 really hard to find a signal 00:07:14:14 - 00:07:16:09 and then there's no battery. 00:07:16:09 - 00:07:17:22 And and even if you're out there 00:07:17:22 - 00:07:22:18 and there is a signal based on how the 00:07:22:20 - 00:07:25:12 location works with triangulating 00:07:25:12 - 00:07:26:13 towers, 00:07:26:13 - 00:07:29:13 the accuracy of that location is 00:07:29:18 - 00:07:30:22 not the accuracy 00:07:30:22 - 00:07:33:13 you get with Uber or with your, 00:07:33:13 - 00:07:35:06 Google Maps, 00:07:35:06 - 00:07:36:20 or those opt-in apps. 00:07:36:20 - 00:07:39:16 So it is less accurate. 00:07:39:16 - 00:07:42:06 So if I were to try to rescue 00:07:42:06 - 00:07:43:14 someone based on a cell phone 00:07:43:14 - 00:07:46:14 location versus a satellite position, 00:07:46:14 - 00:07:47:12 ten times out of ten, 00:07:47:12 - 00:07:48:21 satellite position is going to allow me 00:07:48:21 - 00:07:50:03 to find them faster. 00:07:50:03 - 00:07:53:01 Oh, so that is, on all weather advice, 00:07:53:01 - 00:07:53:21 I think. 00:07:53:21 - 00:07:54:15 But in regards 00:07:54:15 - 00:07:55:07 to once 00:07:55:07 - 00:07:56:09 we're dealing with snow 00:07:56:09 - 00:07:58:16 and ice and medium-angle 00:07:58:16 - 00:08:01:11 or adjacent medium-angle terrain, 00:08:01:11 - 00:08:03:12 you really need to have crampons. 00:08:03:12 - 00:08:04:11 You need to have an ice 00:08:04:11 - 00:08:06:12 ax to arrest a fall, 00:08:06:12 - 00:08:10:04 a helmet, and you need to have time 00:08:10:06 - 00:08:12:01 spent using that equipment 00:08:12:01 - 00:08:13:15 in a non-life-threatening event. 00:08:13:15 - 00:08:14:22 So you can't just or 00:08:14:22 - 00:08:15:18 you wouldn't recommend 00:08:15:18 - 00:08:16:12 someone just order 00:08:16:12 - 00:08:19:04 all this stuff and go do it. No. 00:08:19:04 - 00:08:20:14 It's like anything else in life. 00:08:20:14 - 00:08:21:05 Yeah, right. 00:08:21:05 - 00:08:22:04 You gotta 00:08:22:04 - 00:08:23:08 we all start bad at things, 00:08:23:08 - 00:08:25:00 we practice, and we get better. Yeah. 00:08:25:00 - 00:08:25:09 Right. Yeah. 00:08:25:09 - 00:08:26:16 So how do you practice? Where do you. 00:08:26:16 - 00:08:27:00 How do you 00:08:27:00 - 00:08:27:20 how do you start using this? 00:08:27:20 - 00:08:28:17 So you can use that 00:08:28:17 - 00:08:29:15 equipment in an area 00:08:29:15 - 00:08:32:09 that is low angle to get used to it. 00:08:32:09 - 00:08:33:10 Medium angle 00:08:33:10 - 00:08:34:22 with no consequences 00:08:34:22 - 00:08:36:07 if you lose control. 00:08:36:07 - 00:08:37:03 Those are types, areas 00:08:37:03 - 00:08:38:15 we can train on it. Yeah. 00:08:38:15 - 00:08:39:04 And I think 00:08:39:04 - 00:08:40:15 one thing that I think 00:08:40:15 - 00:08:43:03 makes a lot of people nervous is 00:08:43:03 - 00:08:45:17 the use of micro spikes. 00:08:45:19 - 00:08:47:07 Micro spikes are 00:08:47:07 - 00:08:48:05 a traction 00:08:48:05 - 00:08:48:14 device 00:08:48:14 - 00:08:51:22 you can put on essentially any shoe, 00:08:52:00 - 00:08:54:17 and they may give a false sense 00:08:54:17 - 00:08:57:12 of security when compared to crampons. 00:08:57:12 - 00:09:01:02 Crampons are very sharp. 00:09:01:04 - 00:09:03:15 Very large traction device 00:09:03:15 - 00:09:04:12 that attaches 00:09:04:12 - 00:09:07:12 to a compatible mountaineering boot. 00:09:07:13 - 00:09:09:09 And they give a much better grip 00:09:09:09 - 00:09:10:19 than micro spikes. 00:09:10:19 - 00:09:12:04 Micro spikes are okay 00:09:12:04 - 00:09:13:20 walking down the sidewalk 00:09:13:20 - 00:09:15:20 in the Midwest after a storm.
