00:00:00:02 - 00:00:02:04 Welcome to Live Well with UCI Health. 00:00:02:04 - 00:00:04:08 I'm Ryan Manno from KOST 103.5 00:00:04:08 - 00:00:05:06 and iHeartRadio. 00:00:05:06 - 00:00:06:06 Very pleased to be joined today 00:00:06:06 - 00:00:07:11 by Doctor Shalini Shah. 00:00:07:11 - 00:00:09:04 I want to get your title Vice chair 00:00:09:04 - 00:00:10:23 of the Department of Anesthesiology 00:00:10:23 - 00:00:12:19 and Director of Pain Services 00:00:12:19 - 00:00:14:12 at UCI Health Center 00:00:14:12 - 00:00:15:07 for Pain and Wellness. 00:00:15:07 - 00:00:16:20 Doctor Shah, thank you for being here. 00:00:16:20 - 00:00:17:08 My pleasure. 00:00:17:08 - 00:00:18:16 So let's talk about pain 00:00:18:16 - 00:00:19:19 and not the kind that comes 00:00:19:19 - 00:00:21:18 with waking up every day. 00:00:21:18 - 00:00:23:02 Like the, you know, physical pain. 00:00:23:02 - 00:00:26:01 And I think it's probably safe to say 00:00:26:07 - 00:00:28:21 that most people, as we age, 00:00:28:21 - 00:00:31:01 deal with pain pretty regularly, 00:00:31:01 - 00:00:33:03 if not constantly. 00:00:33:03 - 00:00:33:17 Yeah. 00:00:33:17 - 00:00:34:01 You know, 00:00:34:01 - 00:00:36:00 the way we think about pain in 00:00:36:00 - 00:00:38:04 right now in the country is episodic. 00:00:38:04 - 00:00:39:07 It happens for a small time. 00:00:39:07 - 00:00:40:22 I hurt my knee; I sprained my ankle. 00:00:40:22 - 00:00:42:20 But there's a whole segment of people 00:00:42:20 - 00:00:44:09 who are living with pain chronically. 00:00:44:09 - 00:00:46:07 It's a part of their everyday life. 00:00:46:07 - 00:00:47:10 It's going to be a part of who 00:00:47:10 - 00:00:48:23 we are as we age, and that's a fact. 00:00:48:23 - 00:00:50:23 And so how do we get ahead of it 00:00:50:23 - 00:00:51:11 is really what 00:00:51:11 - 00:00:53:05 we're focusing on here at UCI. 00:00:53:05 - 00:00:55:00 First, let's talk about the difference. 00:00:55:00 - 00:00:55:15 There was a question, 00:00:55:15 - 00:00:57:01 there was some terms on here 00:00:57:01 - 00:00:58:20 that I wanted to bring up to you 00:00:58:20 - 00:01:00:02 the difference between lifespan 00:01:00:02 - 00:01:01:05 and health span. 00:01:01:05 - 00:01:02:11 What is that? Yeah. 00:01:02:11 - 00:01:04:05 So we hear a lot of these buzzwords. 00:01:04:05 - 00:01:05:08 We hear the word 00:01:05:08 - 00:01:06:07 lifespan, that's just 00:01:06:07 - 00:01:07:20 how long we're going to live. 00:01:07:20 - 00:01:10:00 And then we talk more about health span. 00:01:10:00 - 00:01:12:01 Is those lives that matter 00:01:12:01 - 00:01:12:17 or those years 00:01:12:17 - 00:01:14:06 that matter, the quality years. 00:01:14:06 - 00:01:15:08 And I'll tell you, 00:01:15:08 - 00:01:15:15 you know, 00:01:15:15 - 00:01:16:20 just yesterday in clinic, 00:01:16:20 - 00:01:18:07 I saw probably seven 00:01:18:07 - 00:01:20:03 out of my entire panel 00:01:20:03 - 00:01:21:13 for the day were patients 00:01:21:13 - 00:01:22:09 who are 90 plus, 00:01:22:09 - 00:01:23:23 and those patients are 00:01:23:23 - 00:01:25:22 going to easily touch 100 00:01:25:22 - 00:01:27:01 without any difficulty. 00:01:27:01 - 00:01:28:21 So people are living longer. Right. 00:01:28:21 - 00:01:30:11 And they're healthy, to be honest. 00:01:30:11 - 00:01:32:19 They're living longer, healthier lives. 00:01:32:19 - 00:01:34:02 And so how do you get to be 00:01:34:02 - 00:01:34:16 that one 00:01:34:16 - 00:01:36:00 of those special types of people, 00:01:36:00 - 00:01:36:13 you know, 00:01:36:13 - 00:01:37:21 where you're able to manage, 00:01:37:21 - 00:01:38:21 you have a little bit of pain 00:01:38:21 - 00:01:40:07 and you keep that pain episodic 00:01:40:07 - 00:01:42:03 and not chronic. So how do you. 00:01:44:06 - 00:01:44:23 Here we go. 00:01:44:23 - 00:01:45:11 Yeah. 00:01:45:11 - 00:01:47:05 So typically so as I'm understanding it 00:01:47:05 - 00:01:50:05 your, your health span will almost always 00:01:50:05 - 00:01:51:16 be shorter than your lifespan. 00:01:51:16 - 00:01:52:04 Yeah. Right. 00:01:52:04 - 00:01:54:00 For most people. Really well said. Yes. 00:01:54:00 - 00:01:54:23 Is that right. Yes. Okay. 00:01:54:23 - 00:01:56:21 So how then do we. 00:01:56:21 - 00:01:58:03 Maybe you don't have exactly the answer. 00:01:58:03 - 00:01:59:03 Because if you did, you know 00:01:59:03 - 00:02:00:16 then everyone would be wanting to know 00:02:00:16 - 00:02:03:06 what can we do preventatively 00:02:03:06 - 00:02:05:12 to give ourselves a good shot 00:02:05:12 - 00:02:06:19 at a nice health span? 00:02:06:19 - 00:02:07:21 If we look at it, 00:02:07:21 - 00:02:09:16 I like to say it in 360 view, 00:02:09:16 - 00:02:11:13 and I don't want to get too nerdy on. 00:02:11:13 - 00:02:13:21 This topic. No, no. Nerd out. Let’s go. 00:02:13:21 - 00:02:15:10 There's some metabolic factors, just, 00:02:15:10 - 00:02:15:16 you know, 00:02:15:16 - 00:02:17:08 insulin resistance, visceral fat, 00:02:17:08 - 00:02:19:06 things like that. Muscle. 00:02:19:06 - 00:02:21:00 And we'll talk about muscle in a second. 00:02:21:00 - 00:02:24:04 The second is some biological markers 00:02:24:04 - 00:02:25:04 that most people don't know, that 00:02:25:04 - 00:02:26:09 they're going to the gym every day. 00:02:26:09 - 00:02:26:22 And they don't 00:02:26:22 - 00:02:28:18 know that these are the markers 00:02:28:18 - 00:02:30:16 that predict a long life. 00:02:30:16 - 00:02:31:18 And that's grip strength. 00:02:31:18 - 00:02:32:05 So if you do 00:02:32:05 - 00:02:33:15 grip strength at the gym 00:02:33:15 - 00:02:34:20 and you measure that, you can even buy 00:02:34:20 - 00:02:36:02 a tool off of Amazon 00:02:36:02 - 00:02:36:16 to measure that 00:02:36:16 - 00:02:37:21 and to see if you can improve that 00:02:37:21 - 00:02:38:21 and hanging 00:02:38:21 - 00:02:41:21 if you can hang even for five seconds, 00:02:41:23 - 00:02:42:09 that will. 00:02:42:09 - 00:02:43:12 That's a great marker 00:02:43:12 - 00:02:44:20 to tell you how long you will live 00:02:44:20 - 00:02:46:23 and how the quality of life would be. 00:02:46:23 - 00:02:49:05 Hanging. Hanging. And gripping. 00:02:49:05 - 00:02:50:18 That is so interesting. 00:02:50:18 - 00:02:51:17 Yeah, and that's apart 00:02:51:17 - 00:02:53:00 from all the metabolic factors, 00:02:53:00 - 00:02:53:11 you know, 00:02:53:11 - 00:02:54:11 what we put in our system 00:02:54:11 - 00:02:55:20 and how our metabolic rate runs. 00:02:55:20 - 00:02:57:02 But yeah. Wow. 00:02:58:01 - 00:02:58:13 Let's talk 00:02:58:13 - 00:02:59:20 about some of that metabolic stuff. 00:02:59:20 - 00:03:00:21 Like what 00:03:00:21 - 00:03:01:10 what do you 00:03:01:10 - 00:03:02:13 what do you mean when you say that. 00:03:02:13 - 00:03:04:05 Can you go a little further on that? Yeah. 00:03:04:05 - 00:03:05:06 So if you're in pain 00:03:05:06 - 00:03:06:07 you're probably not moving 00:03:06:07 - 00:03:07:15 because it hurts to move. 00:03:07:15 - 00:03:08:13 So you're putting on weight 00:03:08:13 - 00:03:09:18 and that's visceral fat. 00:03:09:18 - 00:03:11:16 So that deep layer of fat 00:03:11:16 - 00:03:13:06 and that is a huge marker 00:03:13:06 - 00:03:15:05 for why we age so fast. Okay. 00:03:15:05 - 00:03:17:19 Because that fat leads to inflammation. 00:03:17:19 - 00:03:20:14 And so inflammation, visceral fat, 00:03:20:14 - 00:03:21:15 these are all the metabolic 00:03:21:15 - 00:03:23:04 things that limit 00:03:23:04 - 00:03:26:06 how fast we can process either 00:03:26:06 - 00:03:28:08 foods or metabolism, things like that. 00:03:28:08 - 00:03:29:15 And then so for you 00:03:29:15 - 00:03:31:07 and what you specialize in 00:03:31:07 - 00:03:34:17 then these things are related to pain. 00:03:34:18 - 00:03:36:20 100%. Okay. Yeah. 00:03:36:20 - 00:03:38:17 That's our goal. It's yes you have pain. 00:03:38:17 - 00:03:40:22 We will all have pain. 00:03:40:22 - 00:03:43:21 But how do you work past that to keep 00:03:43:21 - 00:03:46:00 those metabolic markers strong? 00:03:46:00 - 00:03:48:06 You know you mentioned chronic pain. 00:03:48:06 - 00:03:50:03 What separates a pain? 00:03:50:03 - 00:03:51:20 Just like a nagging pain versus 00:03:51:20 - 00:03:53:18 like when does a pain become chronic? 00:03:53:18 - 00:03:55:08 My definition is anything longer 00:03:55:08 - 00:03:57:01 than six weeks to three months, right? 00:03:57:01 - 00:03:57:17 Okay. 00:03:57:17 - 00:03:59:12 Most commonly, the number one reason 00:03:59:12 - 00:04:01:00 why people go and seek a doctor 00:04:01:00 - 00:04:03:07 for pain is typically low back pain. 00:04:03:07 - 00:04:04:23 And that's about 70 to 80% 00:04:04:23 - 00:04:07:07 of what we do and treat is low back pain. 00:04:07:07 - 00:04:09:18 And to be honest, that's the space. 00:04:09:18 - 00:04:11:11 If people want promise in the world, 00:04:11:11 - 00:04:12:13 that's the one space 00:04:12:13 - 00:04:13:12 that's really improving. 00:04:13:12 - 00:04:15:07 A lot of new technologies coming out, 00:04:15:07 - 00:04:16:11 a lot of noninvasive. 00:04:16:11 - 00:04:18:05 We used to think we need spine surgery. 00:04:18:05 - 00:04:18:21 There's a lot of things 00:04:18:21 - 00:04:20:19 that you may not need now. 00:04:20:19 - 00:04:22:08 So that space is really improving. 00:04:22:08 - 00:04:23:01 And so you're going to see 00:04:23:01 - 00:04:24:10 a lot more treatments available 00:04:24:10 - 00:04:26:11 to take care of that. 00:04:26:11 - 00:04:27:02 The part 00:04:27:02 - 00:04:30:01 that is harder to treat is frailty. 00:04:30:01 - 00:04:33:01 So as we age we get frail. 00:04:33:01 - 00:04:35:08 And that means we're losing muscle mass. 00:04:35:08 - 00:04:36:06 And as we lose muscle 00:04:36:06 - 00:04:37:09 mass, we're not moving as much. 00:04:37:09 - 00:04:39:07 We can't support our skeleton. 00:04:39:07 - 00:04:40:05 It's painful. 00:04:40:05 - 00:04:42:00 And then you see the hip fractures. 00:04:42:00 - 00:04:42:15 And then once you get 00:04:42:15 - 00:04:43:08 in the hip fracture, 00:04:43:08 - 00:04:44:09 then you go to the hospital, 00:04:44:09 - 00:04:45:20 and then you need a convalescent home 00:04:45:20 - 00:04:47:18 and all that entire cascade... Yes. 00:04:47:18 - 00:04:48:18 that we see with our parents 00:04:48:18 - 00:04:50:18 and our grandparents. You know. Yeah. 00:04:50:18 - 00:04:52:00 And that's what we're trying to break. 00:04:52:00 - 00:04:53:12 And so a lot of what 00:04:53:12 - 00:04:55:04 we're trying to teach our patients 00:04:55:04 - 00:04:57:08 is about building that muscle mass. 00:04:57:08 - 00:05:00:06 So after the age of 30, we lose 00:05:00:06 - 00:05:02:15 about 3 to 5% of our muscle mass 00:05:02:15 - 00:05:04:16 every decade. Just that's normal. 00:05:04:16 - 00:05:05:23 We will all do that. 00:05:05:23 - 00:05:09:01 But after 60, we start to lose about 00:05:09:01 - 00:05:12:11 15 to 20% of our muscle every year. 00:05:12:11 - 00:05:14:10 And I've seen those patients 00:05:14:10 - 00:05:16:03 who've lived that to that 90 00:05:16:03 - 00:05:18:16 year mark or 85, 90, 95 year mark, 00:05:18:16 - 00:05:19:21 they still have preserved muscle. 00:05:19:21 - 00:05:21:17 They're still going to gym every day 00:05:21:17 - 00:05:23:02 as much as they can to 00:05:23:02 - 00:05:24:12 to keep gaining that muscle 00:05:24:12 - 00:05:27:08 so they don't become frail. Yeah. 00:05:27:08 - 00:05:29:11 And would you say that if, if you had to 00:05:29:11 - 00:05:31:15 just, you know, your own, 00:05:32:16 - 00:05:34:06 not your own research, but your own 00:05:34:06 - 00:05:35:06 the things you've observed, 00:05:35:06 - 00:05:36:06 your own observations, 00:05:36:06 - 00:05:36:13 is that 00:05:36:13 - 00:05:38:04 is that a commonality in those people 00:05:38:04 - 00:05:40:02 that have not only the long lifespan, 00:05:40:02 - 00:05:41:22 but the long health span 00:05:41:22 - 00:05:44:01 is that muscle mass, is going to the gym? 00:05:44:01 - 00:05:44:13 It is. 00:05:44:13 - 00:05:45:20 Yeah, almost. 00:05:45:20 - 00:05:47:11 One of the things that can correlate 00:05:47:11 - 00:05:50:03 so truly is that muscle mass. 00:05:50:03 - 00:05:53:05 I wish we did a better job in nutrition 00:05:53:05 - 00:05:54:08 or teaching medical students 00:05:54:08 - 00:05:55:01 about nutrition, 00:05:55:01 - 00:05:56:07 teaching our physicians 00:05:56:07 - 00:05:56:22 about nutrition 00:05:56:22 - 00:05:58:14 to educate our patients 00:05:58:14 - 00:05:59:22 and about these things 00:05:59:22 - 00:06:01:00 because they're simple. 00:06:01:00 - 00:06:02:09 They're not complicated, 00:06:02:09 - 00:06:04:04 simple things that we could do. 00:06:04:04 - 00:06:06:11 And if you if you do them, 00:06:06:11 - 00:06:07:16 my dad always preaches this. 00:06:07:16 - 00:06:09:21 I mean, he's 78 and he still goes. 00:06:09:21 - 00:06:10:19 He swims every morning. 00:06:10:19 - 00:06:11:20 He's at the gym every morning. 00:06:11:20 - 00:06:14:06 And he's always telling me, 00:06:14:06 - 00:06:15:06 get ahead of it now 00:06:15:06 - 00:06:16:12 or make it part of your life now. 00:06:16:12 - 00:06:17:16 Because then when you 00:06:17:16 - 00:06:18:19 when you get older 00:06:18:19 - 00:06:19:11 and you just try to 00:06:19:11 - 00:06:20:12 play catch up with your health 00:06:20:12 - 00:06:21:00 and it's like, 00:06:21:00 - 00:06:22:16 but you could have been doing that 00:06:22:16 - 00:06:23:11 a long time ago. 00:06:23:11 - 00:06:24:01 Yeah, you could have 00:06:24:01 - 00:06:25:06 done the preventative stuff. 00:06:25:06 - 00:06:26:04 Yes, yes. Right. 00:06:26:04 - 00:06:26:23 And so starting it 00:06:26:23 - 00:06:27:21 now, even, 00:06:27:21 - 00:06:28:01 you know, 00:06:28:01 - 00:06:29:06 no matter how old you are, 00:06:29:06 - 00:06:31:00 it's, it's never too late to start. 00:06:31:00 - 00:06:31:08 Yeah. 00:06:31:08 - 00:06:32:23 And it's never too early to incorporate 00:06:32:23 - 00:06:34:14 a lot more protein into your diet, 00:06:34:14 - 00:06:38:13 you know, for a healthy 40 to 50 year 00:06:38:13 - 00:06:39:00 old female, 00:06:39:00 - 00:06:40:15 taking about 100g of protein 00:06:40:15 - 00:06:42:01 a day is very easy. 00:06:42:01 - 00:06:44:04 It's a very good thing to do. Yeah. 00:06:44:04 - 00:06:45:10 To build that muscle mass. 00:06:45:10 - 00:06:46:15 And it's interesting too, 00:06:46:15 - 00:06:47:14 because I think when 00:06:47:14 - 00:06:48:20 when you're not thinking about it, 00:06:48:20 - 00:06:49:19 you're just going through your day. 00:06:49:19 - 00:06:50:15 You're not thinking about 00:06:50:15 - 00:06:51:17 how those things correlate 00:06:51:17 - 00:06:53:03 and then lead to pain. 00:06:53:03 - 00:06:54:07 And they're coming to see you going, 00:06:54:07 - 00:06:55:05 well, 00:06:55:05 - 00:06:55:19 you know, 00:06:55:19 - 00:06:57:23 my lower back is messed up, by the way. 00:06:57:23 - 00:06:59:09 Why is it like 00:07:00:15 - 00:07:01:03 why can't. 00:07:01:03 - 00:07:02:17 Like if I think about sleeping on like, 00:07:02:17 - 00:07:03:16 we had sleepovers as kids 00:07:03:16 - 00:07:04:11 and we would all just, like, 00:07:04:11 - 00:07:05:18 sleep on the floor 00:07:05:18 - 00:07:07:10 and like nothing happened, right? 00:07:07:10 - 00:07:08:08 We, we have a sleep 00:07:08:08 - 00:07:09:03 number bed at home, 00:07:09:03 - 00:07:10:23 and my side has a leak in it, and, 00:07:10:23 - 00:07:11:23 like, it'll sag a little. 00:07:11:23 - 00:07:13:13 And I swear those mornings when it 00:07:13:13 - 00:07:15:03 when I'm, I'm like, 00:07:15:03 - 00:07:17:11 what did I get in a fight? 00:07:17:11 - 00:07:18:22 My whole body is like, 00:07:18:22 - 00:07:20:23 you know, and I'm 45. Yeah. 00:07:20:23 - 00:07:22:15 But we hit a point 00:07:22:15 - 00:07:25:08 where even the smallest little difference 00:07:25:08 - 00:07:28:21 in our day to day can cause pain, 00:07:28:21 - 00:07:30:00 and it can ruin your day. 00:07:30:00 - 00:07:31:22 It can. You tweak that muscle wrong. 00:07:31:22 - 00:07:33:16 Usually it's muscle, thank God. 00:07:33:16 - 00:07:35:07 But yeah, you're absolutely right. 00:07:35:07 - 00:07:37:01 You feel it. You're feeling it. Yes. 00:07:37:01 - 00:07:37:06 Right. 00:07:37:06 - 00:07:38:02 You're feeling it 00:07:38:02 - 00:07:39:04 where the discs are 00:07:39:04 - 00:07:40:12 not as supportive as they are 00:07:40:12 - 00:07:41:12 or they once were. 00:07:41:12 - 00:07:42:20 So you need more support from 00:07:42:20 - 00:07:44:02 from a firm mattress. 00:07:44:02 - 00:07:45:12 Yeah. We feel it. Yeah. 00:07:45:12 - 00:07:46:10 And it really does. 00:07:46:10 - 00:07:49:09 It inhibits your enjoyment of life. 00:07:49:09 - 00:07:49:21 You know, 00:07:49:21 - 00:07:50:12 when something's 00:07:50:12 - 00:07:53:07 nagging at you or worse than nagging. 00:07:53:07 - 00:07:54:17 What are some of the ways 00:07:54:17 - 00:07:55:23 pain is treated today. 00:07:55:23 - 00:07:57:05 So you're probably going to hear 00:07:57:05 - 00:07:58:17 the word often multimodal. 00:07:58:17 - 00:08:00:07 And what that means is 00:08:00:07 - 00:08:02:13 many ways at the same time. 00:08:02:13 - 00:08:05:11 So it's not just prescription painkillers 00:08:05:11 - 00:08:06:10 or medications. 00:08:06:10 - 00:08:07:21 It's not just physical therapy, 00:08:07:21 - 00:08:09:08 but it's everything together. 00:08:09:08 - 00:08:10:23 So that's what we call multimodal. 00:08:10:23 - 00:08:11:22 We try to treat it 00:08:11:22 - 00:08:13:06 aggressively through 00:08:13:06 - 00:08:14:22 many means as fast as we can, 00:08:14:22 - 00:08:16:01 because we need you to get back 00:08:16:01 - 00:08:17:16 moving and building that muscle mass. 00:08:17:16 - 00:08:18:08 Yep. Really? 00:09:49:01 - 00:09:50:06 Seeing a lot more mental health. 00:09:50:06 - 00:09:52:04 And that's one of the biggest barriers 00:09:52:04 - 00:09:52:23 to overcome 00:09:52:23 - 00:09:54:16 when we're trying to treat pain. 00:09:54:16 - 00:09:55:18 Talk about that. Why? 00:09:55:18 - 00:09:56:18 How? What do you mean? 00:09:56:18 - 00:09:57:01 You know, 00:09:57:01 - 00:10:00:01 I don't know if it's generational or not, 00:10:00:02 - 00:10:02:01 but we're seeing younger folks 00:10:02:01 - 00:10:03:14 and I mean 20s, 30s, 40s 00:10:03:14 - 00:10:05:19 who are coming into us as patients 00:10:05:19 - 00:10:06:19 with pain, 00:10:06:19 - 00:10:07:23 who also have 00:10:07:23 - 00:10:09:11 considerable amount of anxiety 00:10:09:11 - 00:10:10:15 that they're dealing with, 00:10:10:15 - 00:10:13:01 whether it's workplace stress, 00:10:13:01 - 00:10:14:02 coping skills, 00:10:14:02 - 00:10:15:10 and I don't know if it's a generation 00:10:15:10 - 00:10:17:05 where they were the cell phone generation 00:10:17:05 - 00:10:18:05 where they to cope, 00:10:18:05 - 00:10:19:20 they went to the cell phone or, 00:10:19:20 - 00:10:21:06 you know, sometimes we 00:10:22:05 - 00:10:23:12 just scroll and that's the 00:10:23:12 - 00:10:25:10 way we deal with our stress. 00:10:25:10 - 00:10:26:18 So but how does that then... 00:10:26:18 - 00:10:28:07 I’m fascinated by this. 00:10:28:07 - 00:10:29:18 How does that then 00:10:29:18 - 00:10:31:05 get in the way of what it is 00:10:31:05 - 00:10:32:06 you're trying to treat? 00:10:32:06 - 00:10:33:04 Oh great question. 00:10:33:04 - 00:10:34:12 So what most people don't know 00:10:34:12 - 00:10:37:10 is that pain, anxiety and depression 00:10:37:10 - 00:10:39:00 travel together. Really. 00:10:39:00 - 00:10:40:14 And they synapse at the same places 00:10:40:14 - 00:10:42:16 in the brain together. 00:10:42:16 - 00:10:44:21 So if you have pain, likely 00:10:44:21 - 00:10:45:17 you will develop 00:10:45:17 - 00:10:46:08 chronic pain, 00:10:46:08 - 00:10:46:19 I mean, 00:10:46:19 - 00:10:48:06 and some element of anxiety 00:10:48:06 - 00:10:50:00 and depression and vice versa. 00:10:50:00 - 00:10:51:21 So if you have anxiety and depression, 00:10:51:21 - 00:10:54:08 you will also at some point develop pain. 00:10:54:08 - 00:10:56:17 And so there's what we call comorbid. 00:10:56:17 - 00:10:58:03 They exist together. 00:10:58:03 - 00:10:59:10 So if I'm trying to treat pain 00:10:59:10 - 00:11:00:19 in someone who's anxious 00:11:00:19 - 00:11:02:12 or who's severely depressed, 00:11:02:12 - 00:11:03:12 it's very difficult. 00:11:03:12 - 00:11:04:00 I got 00:11:04:00 - 00:11:05:06 I have to treat that anxiety 00:11:05:06 - 00:11:06:23 and depression component first 00:11:06:23 - 00:11:08:18 because they travel together. 00:11:08:18 - 00:11:10:15 Yeah. Yes. Yes. 00:13:19:02 - 00:13:20:09 Look at you, doctor. 00:13:20:09 - 00:13:23:00 So he has, like, he was on set, and he... 00:13:23:00 - 00:13:24:04 He put out this video on Instagram 00:13:24:04 - 00:13:25:19 the other day where he was saying he 00:13:25:19 - 00:13:27:11 he did something he, like, 00:13:27:11 - 00:13:28:22 tore something in his, 00:13:28:22 - 00:13:30:03 his quad or something. 00:13:30:03 - 00:13:31:05 He's going to physical therapy 00:13:31:05 - 00:13:32:10 throughout his back 00:13:32:10 - 00:13:33:18 led to an ankle sprain. 00:13:33:18 - 00:13:35:06 Like it was boom, boom, boom. 00:13:35:06 - 00:13:37:02 And he said, as I've gotten older, 00:13:37:02 - 00:13:38:08 I have learned that 00:13:38:08 - 00:13:40:09 if you want to live right, 00:13:40:09 - 00:13:41:19 you got to keep your glutes tight. 00:13:41:19 - 00:13:43:02 And so he's like, when I walk, 00:13:43:02 - 00:13:44:09 I squeeze my ass. 00:13:44:09 - 00:13:46:21 And it's helped me prevent 00:13:46:21 - 00:13:48:09 so many other things. 00:13:48:09 - 00:13:49:00 So he was just like, 00:13:49:00 - 00:13:49:17 when you walk, squeeze 00:13:49:17 - 00:13:50:07 your butt cheeks, 00:13:50:07 - 00:13:52:14 just clench your butt and build up that 00:13:52:14 - 00:13:53:17 that muscle mass. Yeah. 00:14:28:18 - 00:14:30:16 One of the 00:14:30:16 - 00:14:32:18 strongest thread points that we see. 00:14:32:18 - 00:14:34:00 And this is not just my patients, 00:14:34:00 - 00:14:35:20 I'm saying from data sets 00:14:35:20 - 00:14:37:03 is that they have 00:14:37:03 - 00:14:39:07 some element of meditation 00:14:39:07 - 00:14:42:07 or some element of non-reactivity. 00:14:42:07 - 00:14:44:10 They're not reactive in life. 00:14:44:10 - 00:14:45:09 They're not letting things 00:14:45:09 - 00:14:47:03 get to them on a daily basis, 00:14:47:03 - 00:14:48:17 an hourly basis. 00:14:48:17 - 00:14:50:11 They've got that mental. 00:14:50:11 - 00:14:53:08 I don't know the strength to maintain 00:14:53:08 - 00:14:54:16 a non-reactive environment. 00:14:54:16 - 00:14:56:16 And that's also really important 00:14:56:16 - 00:14:57:18 for a health span. 00:14:57:18 - 00:14:59:20 Sure, it can be hard to do, 00:14:59:20 - 00:15:00:15 especially with everything 00:15:00:15 - 00:15:02:02 coming at us at all times, you know, 00:15:02:02 - 00:15:02:21 and you can try 00:15:02:21 - 00:15:03:22 and you can make it a point 00:15:03:22 - 00:15:06:00 to not engage a reactive mind, 00:15:06:00 - 00:15:07:22 but sometimes it'll it'll hit. Whoa. 00:16:25:08 - 00:16:26:13 and the brain adapts to pain signals 00:16:26:13 - 00:16:27:23 that are coming from the spinal cord. 00:16:27:23 - 00:16:30:04 That's crazy in the coolest way.
