Making Benefit Changes
New employees and employees who have been appointed into a position that provides more benefits should use At Your Service Online to enroll into benefits.
Employees have 31 days from their date of hire or change in appointment to enroll.
Family Status Change
If there has been a change in your family status (marriage, divorce, birth, etc.) and involuntary loss of coverage, complete the UPAY 850 form ›
The UPAY 850 form must be submitted within 31 days of the life event to initiate a benefit change. This type of change cannot be completed through At Your Service.
Fax the completed UPAY 850 form to Human Resources at 855-251-6438 or mail it to Route 114.
After submitting the form, verify enrollment status by logging into At Your Service and selecting "Current Enrollments" from the Health & Welfare menu.
Period of Initial Eligibility (PIE)
Your PIE starts on the first day of eligibility. The following events will qualify as a PIE:
- Newly hired employee
- Change in appointment status
- Newly added family members (spouse, birth, adoption)
- Involuntary loss of coverage
The PIE ends 31 days later. If the 31st day falls on a weekend, the eligibility period ends the next work day.
