Our network includes more than 600 primary care and specialty physicians, plus several trusted community hospitals. We’re committed to delivering high-quality medical care and wellness support for individuals, families and seniors, right in your neighborhood.

If you don't have a healthcare insurance broker and would like to be connected with one, please call 562-534-6161 Monday-Friday, 9 a.m. to 5 p.m. and we'll connect you with an independent licensed agent. You can also enroll at Medicare.gov

Our health plan team provides tools to help provider succeed in value-based programs.

One of those tools is Cozeva, an online integrated population health platform that supports value-based care, quality improvement and risk adjustment performance. By consolidating data from multiple sources into a unified patient record, Cozeva delivers comprehensive patient insights and care opportunities, provides actionable reporting and helps providers monitor quality and risk adjustment metrics in real time through customized dashboards.

Cap Connect

We offer a provider portal, Cap Connect, designed to simplify your workflow. 

Cap Connect is our secure provider portal designed to streamline your workflow and enhance practice efficiency. Through Cap Connect, providers can submit and track referrals, check authorization status, access claims information and more.
