Need a copy of your medical records? UCI Health offers two ways to get them.
MyChart Patient Portal
You may request — and receive — most of your medical records through a UCI Health MyChart patient portal account. This online portal not only gives you access to most of your medical records, it also allows you to schedule appointments, communicate with your care team, request prescription renewals and more.
Note: Some medical records may be available only through our UCI Health Medical Records department.
To obtain a printed copy of your medical records, please follow these steps:
- Download and print an Authorization for Release of Health Information form in English or in Spanish.
- Complete, sign and date the form. In order to verify your identification and validate your authorization, you are required to include a legible copy of a valid photo identification (e.g., a driver’s license, a military ID or a state ID).
- Email, fax, mail or deliver your paperwork in person.
Medical record fees: There is no charge to send your medical records to another healthcare provider. For records released directly to a patient or an authorized family member there is no charge for the first 20 pages. Additional pages are charged at $0.25 per page.

All attorney, copy service and/or subpoena requests must be delivered to the Health Information Management Department.

Please allow seven to 10 business days for processing from date of receipt of the completed authorization. California law permits up to 15 days to respond to requests for medical records.

Urgent Requests/Records for Your Physician
For immediate continuity of care, you or your healthcare providers may request that records to be sent directly to their office.
The physician's office must send a written request on business letterhead to 714-456-7576, indicating the patient's name, date of birth and date of visit. Please write "STAT" at the top of the request. For assistance, call 714-456-5670, press option 5, followed by option 2.
