Due to the COVID-19 outbreak, upcoming Advance Directives classes have been limited to nine (9) people to observe social-distancing requirement. Masks also are required.
Who will speak for you when you can't speak for yourself?
An advance healthcare directive does just that — it tells your healthcare providers what you want to have done (and not done) when you can't speak for yourself.
In this 90-minute class, a doctor and social worker will explain what a healthcare directive form is and how they are used.
We will have forms for you to fill out in class. A notary public will also be available from 12 to 12:30 p.m. to notarize your completed advance healthcare directive form at no charge.
Please bring your family and support persons with you to this important class.
Cost is free to attend. There is a small fee for validated parking at UCI Medical Center.
To register, call 657-282-6357.
