Healthcare Wishes Workshop
Plan ahead and make your voice heard. Join UCI Health and City of Irvine Health and Wellness Department for this free community event where you can formalize your Advanced Healthcare Directive so your loved ones and medical team always know your wishes. A notary public will be on-site to officially finalize your directive, and a licensed clinical social worker will be available to answer questions and provide resources. This free event is open to anyone in the community 18 and older — including patients, caregivers, and loved ones.
What to expect
- Appointments are 30 minutes long
What to bring
- Valid photo ID (driver's license or passport)
- Your completed Advanced Healthcare Directive form
When
The second Tuesday of the month between 9 a.m.-12:30 p.m., and the fourth Tuesday of the month from noon-3:30 p.m.
