Transfer a Patient to a UCI Health Hospital 

Our dedicated Transfer Center team is available 24 hours a day, 7 days a week to coordinate seamless patient transfers and referrals across our six hospitals. With a single call, referring providers gain access to experienced transfer specialists who streamline communication, coordinate logistics, and help connect patients to the highest level of care as quickly and efficiently as possible. 

Our health system serves a broad and diverse community, providing comprehensive acute, specialty, and tertiary care services across multiple hospital locations. From emergent and critical care transfers to advanced specialty and subspecialty services, our hospitals are equipped to support patients with complex medical needs close to home and throughout the region.

To Transfer a Patient, please have the following information ready:  

• H&P 
• Diagnosis 
• Reason for transfer request 
• Referring case manager and referring physician details 

We strongly recommend obtaining access to the UCI Health Transfer Center Portal. Each individual user must their own account to utilize the portal. Once logged in, select the facility and complete the e-intake form. 

For UCIH-Orange, UCIH-Fountain Valley, UCIH-Los Alamitos, and UCIH-Lakewood only:

• Complete the Return Agreement Form.
• If applicable, complete the Letter of Agreement Form.
