UCI Health recently acquired a local health plan, PHS Premier Health Plan Network. Going forward this health plan will be supported by UCI Health under a new name, UCI Health Plan Network. If you are an existing member or physician, you’ll still have access to the same level of support and the same provider network participants. Members will eventually be issued new membership cards with our new name, UCI Health Plan Network, but your existing cards are still valid and accepted by all provider network participants. There is no change in your benefit coverage as a result of our name change to UCI Health Plan Network.

If you don’t currently have a healthcare insurance broker and would like to be connected with one, please call 833-700-9099 (Monday-Friday, 9 a.m. to 5 p.m.), and we’ll connect you with an independent licensed agent. You can also enroll at Medicare.gov.
